Atrial Septal Defektli Olgularda Sistolik Zaman İntervallerinin Değerlendirilmesi

Abstract

Bu çalışma, konjestif kalp yetmezliği bulunmayan, sinüs ritmli ve kardiotonik ilaç kullanmayan 24 ostium sekundum tip atrial septal defektli hastada sol ventrikül fonksiyonunu noninvaziv olarak değerlendirmek amacıyla gerçekleştirilmiştir. Hastalardan simultane EKG, fonokardiyogram ve karotitogram yardımıyla sistolik zaman intervalleri ölçülmüş; preejeksiyon süresi (PEP), sol ventrikül ejeksiyon zamanı (SLVEZ) ve bu iki değerin oranı hesaplanmıştır. Veriler, Weissler formülleri ile kalp hızına göre düzeltilerek analiz edilmiştir. Elde edilen bulgulara göre PEP anlamlı olarak uzamış (p<0.05), SLVEZ ise kısalmış (p<0.05) bulunmuştur. PEP/SLVEZ oranı ise erişkinler için bildirilen normal değerlerin üzerinde saptanmıştır (p<0.01). Bu sonuçlar, sekundum ASD’li hastalarda sol ventrikül kontraktilitesinde bozulma olabileceğini ve sistolik zaman intervallerinin tanı ve izlemde yararlı bir yöntem olabileceğini göstermektedir.

Evaluation of Systolic Time Intervals in Patients with Atrial Septal Defect

Abstract

This study aimed to noninvasively assess left ventricular function in 24 patients with ostium secundum type atrial septal defect who had no signs of congestive heart failure, maintained sinus rhythm, and were not receiving cardiotonic therapy. Systolic time intervals were measured using simultaneous ECG, phonocardiogram, and carotid pulse tracing to determine preejection period (PEP), left ventricular ejection time (LVET), and their ratio. Data were corrected for heart rate using Weissler’s regression equations. Findings revealed a significant prolongation in PEP (p<0.05), a reduction in LVET (p<0.05), and an elevated PEP/LVET ratio compared to normative adult values (p<0.01). These results suggest impaired left ventricular contractility in patients with ASD and indicate that systolic time interval measurements may serve as a useful method for diagnosis and follow-up.
