Allerjik Bronş Astımında İnhalasyon-Provokasyon Testi

A study was conducted on 21 patients with allergic bronchial asthma, all of whom were free from complications, to investigate the effect of inhalation-provocation or bronchial sensitivity tests using specific skin allergens through inhalation. The patients were divided into two groups based on the specific antigens identified via skin tests. The first group received a mixed antigen, while the second group was exposed to house dust antigen in a standard solution (1/1000 w/v). The antigens were inhaled using either an aerosol or an intermittent positive pressure breathing (IPPB) device for 10 minutes. Before the test, control tests with saline inhalation were conducted. After the test, ventilatory function tests were performed to examine the degree of obstruction and the extent of respiratory tract obstruction. In the first group, 13 cases showed a positive result with a 61.5% rate of inhalation-provocation test positivity. The remaining 9 cases showed 15.5% suspicious results, and 23% were negative. In the second group, with house dust antigen, 8 cases showed a 65% positivity rate, 37.5% showed suspicious results, and 12.5% were negative. For the positivity criteria, the Maximum Mid-Expiratory Flow (MMF) index, which is the best and most objective indicator of airway obstruction from various ventilatory tests, was accepted. The literature on this subject was reviewed.

Allerjik bronşial astımda, spesifik cilt alerjenlerinin inhalasyon yolu ile kullanılmasıyla yapılan inhalasyon-provokasyon veya bronşiyal sensitivite testi üzerine bir araştırma yapılmıştır. Bu çalışma için komplikasyonsuz 21 allerjik bronşial astımlı hasta seçilmiştir. Cilt testleri ile tespit edilen spesifik antijenlere göre hastalar iki gruba ayrılmıştır. Birinci gruptakilere karma antijen, ikinci gruptakilere ise ev tozu antijeni w/v oranında 1/1000 standart solüsyon ile 10 dakika süreyle, elpülverizatörü veya IPPB ile inhale ettirilmiştir. Test öncesinde kontrol olarak tuzlu su inhalasyonu yapılmış ve test sonrası vantilatuvar fonksiyon testleri ile solunum yollarındaki obstrüksiyon ve derecesi incelenmiştir. Birinci grupta 13 vakada %61.5 oranında inhalasyon-provokasyon testi pozitif, 9 vakada %15.5 oranında şüpheli ve %23 oranında negatif sonuç bulunmuştur. Ev tozu antijeni ile yapılan testte ise 8 vakada %65 oranında pozitif, %37.5 oranında şüpheli ve %12.5 oranında negatif sonuç elde edilmiştir. Pozitivite kriteri olarak, hava yolu obstrüksiyonunun en iyi ve objektif belirtisi olan Maksimum Orta Ekspirituar Akış Hızı (MMF) endeksi kabul edilmiştir. Konu ile ilgili literatür gözden geçirilmiştir.
