Hypertension Crises in Patients Using Monoamino Oxidase Inhibitors

Abstract

This study presents a case of hypertensive crisis that occurred shortly after the ingestion of yogurt and fava beans in a patient using monoamine oxidase (MAO) inhibitors. While such reactions have traditionally been associated with cheese consumption, this case highlights that similar food-drug interactions may occur with other amine-containing foods. A 92-year-old male experienced a hypertensive emergency with blood pressure rising to 300/190 mmHg, following the intake of a MAO inhibitor (Parnate) and a meal of yogurt and broad beans. The symptoms regressed rapidly following intramuscular administration of reserpine and chlorpromazine. No complications developed. Pathophysiologically, the likely mechanism involves the release of norepinephrine from sympathetic nerve terminals by dietary amines such as tyramine or DOPA, in the context of inhibited MAO activity. This report underscores the importance of dietary restrictions and monitoring in patients treated with MAO inhibitors, extending beyond cheese to include other potentially reactive foods.

Monoami̇no Oksi̇daz İnhi̇bi̇törleri̇ Kullanan Hastalarda Rastlanan Hi̇pertansi̇yon Kri̇zleri̇

Abstract

Bu çalışmada, monoamin oksidaz (MAO) inhibitörü kullanmakta olan bir hastada, yoğurtlu bakla yedikten kısa süre sonra gelişen hipertansif kriz olgusu sunulmuştur. Literatürde genellikle peynir tüketimiyle ilişkilendirilen bu tür krizlerin, yoğurt ve bakla gibi gıdalarla da tetiklenebileceği vurgulanmaktadır. Vaka 92 yaşında bir erkekte ortaya çıkmış, 300/190 mmHg gibi aşırı yüksek kan basıncına rağmen, reserpin ve klorpromazin tedavisi ile semptomlar hızla gerilemiş ve komplikasyon gelişmemiştir. Klinik değerlendirme sonrası, krizin Parnate (tranilsipromin) adlı MAO inhibitörünün kullanımıyla ilişkili olduğu belirlenmiştir. Fizyopatolojik olarak, bakla içerisindeki DOPA ve bozulmuş yoğurt ile alınan potansiyel biyojen aminlerin, MAO enzimi inhibe edildiğinde, noradrenalin salınımını artırarak ani hipertansiyona neden olduğu düşünülmektedir. Bu olgu, MAO inhibitörü kullanan bireylerde yalnızca peynir değil, diğer fermentatif ve amin içeren gıdaların da potansiyel tehlike oluşturabileceğini ortaya koymakta ve dikkatli diyet uyarılarının gerekliliğini vurgulamaktadır.
