Surgical Principles and Outcomes in Secundum Type Atrial Septal Defects

Abstract

This study analyzes 44 cases of secundum type atrial septal defect (ASD) surgically treated between 1974 and 1982 at Ankara University Faculty of Medicine, Department of Thoracic and Cardiac Surgery. The majority of patients were young and presented with significant left-to-right shunting, right ventricular hypertrophy, systolic ejection murmur, and fixed splitting of the second heart sound. Surgical repair was primarily performed using direct sutures; larger defects were closed with Teflon patches. All operations utilized cardiopulmonary bypass and cardioplegic solutions, with no reported mortality. Postoperative complications were minimal, and 90% of the patients resumed normal lives. The findings highlight the effectiveness of early surgical intervention for improved prognosis and lower operative risk.

Sekundum Tipi Atrial Septal Defektlerde Cerrahi Tedavi Prensipleri ve Sonuçları

Abstract

Bu çalışma, 1974–1982 yılları arasında Ankara Üniversitesi Tıp Fakültesi Göğüs ve Kalp Cerrahisi Kliniği’nde cerrahi olarak tedavi edilen 44 sekundum tipi atrial septal defekt (ASD) olgusunu değerlendirmektedir. Hastaların büyük çoğunluğu genç yaş grubundan oluşmakta ve hemodinamik olarak belirgin sağdan sola şant, sağ ventrikül hipertrofisi, sistolik ejeksiyon üfürümü ve sabit çiftleşmiş ikinci kalp sesi bulguları göstermektedir. Cerrahi girişimlerde çoğunlukla primer sütür uygulanırken, daha büyük defektler teflon yama ile kapatılmıştır. Operasyonlar kardiyopulmoner bypass ve kardioplejik solüsyonlarla desteklenmiş; mortalite görülmemiştir. Ameliyat sonrası komplikasyon oranı düşüktür ve hastaların %90'ı normal yaşamlarını sürdürmektedir. Sonuçlar, erken yaşta cerrahi müdahalenin başarı ve prognoz açısından daha olumlu olduğunu göstermektedir.
