Uterus Rüptürleri ve Profilaksisi

1950-1969 yılları arasında Kliniğimizde 12,292 doğum hadisesine karşılık 10 adet uterus rüptürü vak'ası tesbit edilmiştir. Diğer bir deyişle, her 1229 doğumda bir uteruş rüptürü görülmüştür. Rüptür vak'alarından 8’i sezaryen operasyonu, birisi internal version ekstraksiyonu takiben, bir diğeriyse baş-pelvis uygunsuzluğuna bağlı olarak intakt gebe uterusda meydana gelmiştir. Vak'aların 7’si komplet, 3’ü ise inkomplet rüptürlerdir. %10 oranında anne ve %70 oranında çocuk mortalitesi tesbit edilmiştir. Tedavide tıbbi tedavi yanında, 8 vak'ada subtotal histerektomi, 1 vak'ada ise konservatif operasyon uygulanmıştır. Vak'alarımızda elde ettiğimiz bulgular, literatürdekilerle karşılaştırılmış, konu tartışılarak, özellikle uterus rüptürü profilaksisinde varılan sonuçlar belirtilmeye çalışılmıştır.

Between 1950-1969, our clinic observed 10 cases of uterine rupture among 12,292 deliveries. In other words, one uterine rupture occurred for every 1,229 births. Out of the rupture cases, 8 were after cesarean section, one followed internal version extraction, and another occurred due to head-pelvis disproportion in an intact pregnant uterus. Seven of the cases were complete, and three were incomplete ruptures. A maternal mortality rate of 10% and a neonatal mortality rate of 70% were observed. In treatment, in addition to medical treatment, subtotal hysterectomy was performed in 8 cases, and a conservative operation in 1 case. The findings obtained in our cases were compared with the literature, and the topic was discussed, particularly focusing on the prophylaxis of uterine rupture and the results achieved.
