Abstract

SUMMARY
The anterior approach intervention technique used in cervical disc disease and cervical fractures and dislocations is explained. This method, called the Cloward technique, consists of decompressing the spinal cord and nerve roots from the front, then fusing the vertebral bodies with a German bone graft taken from the skull bone.
This surgery is especially useful for hard, chronic discs and medial discs associated with osteophytes, which are also called cervical osteoarthritis or cervical spondylitis, because these discs cannot be reached from the back wall.
This method also holds an important place in cervical fracture dislocations. Since fusion is achieved, there is no need for traction after surgery. Since the hospitalization period is very short, early rehabilitation is also possible.
The anterior approach intervention is a safe and very easy surgery and is performed in a shorter time than the posterior laminectomy.
This method is used to treat tumors that put pressure on the spinal canal.

Abstract

ÖZET
Servikal disk hastalığı ve servikal kırık çıkıklarında kullanılan anterior yaklaşım müdahale tekniği anlatılmaktadır. Cloward tekniği adı verilen bu yöntem, omurilik ve sinir köklerinin ön taraftan dekompresyonu yapıldıktan sonra, kafatası kemiğinden alman Alman kemiği grefti ile omur gövdelerinin füzyonundan ibarettir.
Bu ameliyat özellikle sert, kronik diskler ve osteofitlerle ilişkili medial disklerde, ki bunlara servikal osteoartrit veya servikal spondilit de denir, yararlıdır; çünkü bu disklere arka duvardan ulaşılamaz.
Bu yöntem servikal kırık çıkıklarda da önemli bir yer tutar. Füzyon sağlandığından ameliyattan sonra traksiyona gerek yoktur. Hastanede yatış süresi çok kısa olduğundan erken rehabilitasyon da mümkündür.
Önden yaklaşım müdahalesi güvenli ve çok kolay bir ameliyattır ve arka laminektomiden daha kısa sürede gerçekleştirilir.
Bu yöntem, omurilik kanalına baskı yapan tümörleri tedavi etmek için kullanılır.
