Post-Kolesistektomi Sendromu

Abstract

This study reviews 28 cases of Post-Cholecystectomy Syndrome (PCS) admitted to the Ankara University Department of Gastroenterology between 1960 and 1964. The causes were identified as: residual common bile duct stones (15 cases), unjustified cholecystectomy due to misdiagnosis (10 cases), hepatic insufficiency or cholangitis (4 cases), bile duct injury or stricture (3 cases), and biliary dyskinesia (7 cases). Some patients had multiple complications concurrently. The paper discusses demographic patterns, diagnostic errors, surgical techniques, and outcomes. Most patients had initial surgeries performed outside of university centers, indicating potential shortcomings in diagnostic rigor and operative care. The author emphasizes strict surgical criteria, thorough preoperative evaluation, and the importance of facilities for intraoperative radiologic and pressure diagnostics to minimize PCS.

Post-Cholecystectomy Syndrome: Clinical Review of 28 Cases

Abstract

Bu çalışmada, 1960–1964 yılları arasında Ankara Üniversitesi Gastroenteroloji Kliniği’nde yatarak incelenen 28 Post-Kolesistektomi Sendromu vak’ası değerlendirilmiştir. Olguların 15’inde koledokta taş, 10’unda yanlış teşhisle yapılan ameliyat, 4’ünde hepatit veya kolanjit, 3’ünde safra kanalı yaralanması veya darlığı ve 7’sinde fonksiyonel dissinerji belirlenmiştir. Bazı vak’alarda birden fazla komplikasyon birlikte bulunmuştur. Çoğu hasta üniversite dışındaki hastanelerde ameliyat edilmiş olup, cerrahî endikasyon ve uygulamada ciddi eksiklikler saptanmıştır. Bu tür vak’aların önlenmesi için cerrahî kararlarda titizlik, tecrübeli cerrah ve iyi donanımlı hastaneler tercih edilmesi gerektiği vurgulanmıştır.
