Abstract

This article describes a rare pediatric case of dystonia musculorum deformans emerging after head trauma. Involuntary movements began one month after injury and rapidly generalized over four months. Clinical signs included postural deformities, spastic movements, and speech impairment, with otherwise normal reflexes and lab results. The authors suggest selective extrapyramidal system injury and review literature on pathogenesis and treatment—highlighting surgical options such as chemopallidectomy for symptom relief.

A Case of Dystonia Musculorum Deformans Due to Intracranial Injury

Abstract

Bu makale, 9 yaşındaki bir kız çocuğunda travmadan sonra ortaya çıkan dystonia musculorum deformans hastalığını tanımlamaktadır. Baş travmasından yaklaşık 1 ay sonra sol ayakta başlayan istemsiz distonik hareketler dört ay içinde tüm vücuda yayılmış ve hasta ayağa kalkamaz, yürüyemez hale gelmiştir. Klinik bulgular arasında plantar fleksiyon, ekstremite rotasyonu, lordoz artışı, disartri ve belirgin spazmodik kasılmalar yer almaktadır. Laboratuvar ve görüntüleme bulguları normaldir. Yazarlar, intrakranial travmanın ekstrapiramidal sistemi selektif olarak etkilediğini düşünmekte; bu vakayı literatürdeki benzer olgularla karşılaştırmaktadırlar. Hastalığın tedavisinde ilaçların sınırlı etkisi vurgulanırken, kemopallidektomi gibi cerrahî tekniklerin semptom kontrolünde umut verici olduğu ifade edilmiştir.
