Abstract

This article discusses the use of primary prosthetic replacement in displaced Collum Femoris fractures, particularly when osteosynthesis fails. A clinical review of 5 patients treated over one year is presented, all using Austen-Moore Vitallium prostheses with a posterior approach. Indications for surgery include age over 70, failure of initial internal fixation, pathological fractures, and degenerative joint disease. Approximately 80% success rate was achieved, and complications like infection, embolism, and dislocation were noted. Criteria for assessing outcome include pain, mobility, and gait, with cases classified as excellent, good, fair, or poor.

Abstract

Bu çalışma, deplase Collum Femoris kırıklarının tedavisinde primer protez replasmanının uygulanmasını inceliyor. Başarıyla kaynamayan kırıklarda protez operasyonunun daha etkili bir çözüm sunduğu savunuluyor. Kliniğin bir yıl içindeki 5 vakalık deneyimi aktarılmış; hastaların yaşları 32 ile 88 arasında değişiyor. Uygulanan tüm ameliyatlarda Austen-Moore tipi vitallium protez kullanılmış ve posterior cerrahi yaklaşım tercih edilmiş. Komplikasyonlar arasında enfeksiyon, üremi, pulmoner emboli ve dislokasyon sayılıyor. %80 başarı oranı bildirilmiş, komplikasyon oluşan protezlerin ise çıkarılması öneriliyor. Ayrıca, protez endikasyonları yaş, kırık tipi (özellikle Pauwels III), patolojik fraktürler ve osteoartrit gibi durumlara göre detaylandırılıyor.
