Abstract

Urethral Stricture in Women and Hamartomas of the Kidney Urethral stricture in women is a commonly encountered condition, often underdiagnosed. A clinical review of 1519 female patients revealed 23 cases (1.6%) of urethral stricture with primary symptoms of dysuria, pollakyuria, incontinence, and retention. Pyuria was present in 70% of those cases. Cystoscopic findings varied from normal bladder mucosa to trigonitis and chronic cystitis. Treatment by dilation with Hegar or Dittel bougies performed every 5–7 days proved successful in most cases. Additionally, 20 cases of benign kidney tumors were presented, including hemangiomas, fibromas, adenomas, a leiomyoma, and an adrenal rest. Hamartomas were mostly incidental findings during autopsy or biopsy. Histologic types ranged from cavernous hemangiomas to tubulo-alveolar adenomas. Embryological and pathological implications in the context of tuberous sclerosis were explored.

Abstract

Kadında ürethra darlığı sık rastlanan bir patolojidir. A.Ü. Tıp Fakültesi Üroloji Polikliniğinde iki yıl içinde incelenen 1519 kadın hastadan 23’ünde (%1.6) ürethra darlığı saptanmıştır. Bu hastaların temel şikâyetleri disüri, pollakiüri, enkontinans ve retansiyondur. %70’inde piyüri gözlenmiştir. Sistoskopik muayenelerde mesane mukozası genellikle normal bulunmuş, bazı olgularda trigonit ve kronik sistit saptanmıştır. Tedavide Hegar veya Dittel bujileriyle 5-7 günde bir uygulanan dilatasyon başarı sağlamıştır. Ayrıca benign böbrek tümörlerinden oluşan 20 vaka (7 biyopsi, 13 otopsi) değerlendirilmiştir. Bunlar arasında hemangiom, fibrom, adenom, leiomyom ve sürrenal artığı yer alır. Çoğu olgu otopsi sırasında tesadüfen bulunmuştur. Histolojik olarak kavernöz hemangiom ve tubulo-alveoler adenom gibi farklı morfolojilere sahip lezyonlar rapor edilmiştir. Tüberöz skleroz ile olası ilişkiler embriyolojik ve patolojik açıdan tartışılmıştır.
