Tüberkulotik Plevra Empieması ve Tedavi Yolları

S o n u ç l a r 1 -- Çok defa sun'î pnömotoraks'ın bir ihtilâtı olan tüberkulotik empiema, bu tedavi usulunun yayılması sonucu daha sık görülmçğe başlamıştır. 2- Başlangıçta tıbbî plân tedavi ile bir başarı elde edilmesse cerrahî tedavi geciktirilmemelidir. 3- Saf tuberkulotik empiema'da en elverişli cerrahî tedavi ekstra- plevral torakoplasti'dir. Böyle vak'alarda Kliniğimizin tuttuğu yol şudur : önce, bir kaç oturumda ( çok defa iki, üç. ) yapılan para•vertebral ekstra- plevral torokoplasti ile göğüs duvarı geniş bir sahada kemiksiileştirilir. Empiema boşluğunun oldukca küçülmesine ve hastanın düzelmesine imkân verecek kadar bir zaman geçtikten sora, plevra boşluğu açılarak hasta olan parietal plevra çıkarılır ve visseral plevra küretle temizlenir. Ekstra-plevral torakoplasti'lerle plevra boşluğu dar bir aralık haline geldiğinden açılmasında mahzur yoktur. 4 — Mikst şekillerde iüzumlu olan cerrahî drenaj, saf tüberkulotik empiema'larda kaçınılmaşı gereken tehlikeli bir müdahaledir. 5-Bütün gayretlere rağmen tüberkülo empiema'da mor kalite hâla yüksektir (% 33), ve şifa esas hastalığın sökülüp atılması mânasına değildir. Bununla beraber, doğru yoldan ayrılmamak şartiy:e cerrahî'nin, kaybolmuş sayılan bir çok- hastalara hayatlarını bağışladığıda şüphe götürmez. 6 — En iyi empiema tedavisinin empiema,dan korunma otdugunu söyliyenlerde vardır. Korunma sun'î pnömotoraks vak'alarımn iyi bir şekilde seçilmesine, işe yarar kollaps'a engel olan vissero parietal iltisakların vaktında ayrılmasına, ve iltisakların çöp zülmesi suretiyle tesir göstermeyen bir pnömotoraks'ın erken terkedilmesine bağlıdır.

1 — Tuberculous empyema, often a complication of artificial pneumothorax, has become more frequent due to the widespread use of this treatment method. 2 — If initial medical treatment does not achieve success, surgical treatment should not be delayed. 3 — In pure tuberculous empyema, the most suitable surgical treatment is extra-pleural thoracoplasty. In such cases, our clinic follows this approach: first, the chest wall is ossified over a wide area by performing para-vertebral extra-pleural thoracoplasty in several sessions (often two or three). After sufficient time has passed to allow significant reduction of the empyema cavity and patient improvement, the pleural cavity is opened, the affected parietal pleura is removed, and the visceral pleura is cleaned by curettage. Since extra-pleural thoracoplasties narrow the pleural space to a small interval, opening it poses no problem. 4 — Surgical drainage, which is necessary in mixed forms, is a dangerous intervention that should be avoided in pure tuberculous empyema. 5 — Despite all efforts, mortality in tuberculous empyema remains high (33%), and cure does not mean complete eradication of the primary disease. However, provided the correct approach is maintained, surgery has undoubtedly saved the lives of many patients once considered lost. 6 — Some argue that the best treatment for empyema is prevention. Prevention depends on careful selection of artificial pneumothorax cases, timely separation of visceral-parietal adhesions that prevent effective lung collapse, and early cessation of a pneumothorax that is ineffective due to fibrosis of adhesions.
