Abstract

This paper discusses the anatomical, physiological, and clinical aspects of sweating disorders, focusing on a case of central nervous system–related unilateral hyperhidrosis. Sweat glands are anatomically classified into eccrine and apocrine types, with their autonomic innervation explained—highlighting the functional parasympathetic properties of sympathetic fibers and the neurohormonal control mechanisms of sweating. Sweating disorders are categorized into functional (hyperhidrosis, ephidrosis, bromhidrosis, chromhidrosis, hematidrosis, anhidrosis) and organic types. The presented case involves a patient with post–head trauma left-sided trigeminal distribution hyperhidrosis, hypoesthesia, vestibular hypoexcitability, thermoregulatory, and vasomotor disturbances. Pilocarpine, atropine, and acetylcholine tests objectively evaluated sweating response. Findings suggest an irritative lesion at the pontine level affecting trigeminal sensory nuclei and high intracerebral autonomic centers.

Abstract

Bu çalışma, terleme bozukluklarının anatomik, fizyolojik ve klinik yönlerini ele almakta ve özellikle bir vaka üzerinden santral sinir sistemi kaynaklı unilateral hiperhidrozis bulgularını incelemektedir. Ter bezleri anatomik olarak eccrine ve apocrine tipte sınıflandırılmış; otonom sinir sistemi ile ilişkileri, sempatik liflerin fonksiyonel olarak parasempatik özellik göstermesi ve terlemenin nörohormonal kontrol mekanizmaları detaylı olarak açıklanmıştır. Terleme bozuklukları fonksiyonel (hiperhidroz, ephidroz, bromhidroz, chromhidroz, hematidroz, anhidroz) ve organik olarak iki ana grupta incelenmiştir. Sunulan olgu, geçirilmiş kafa travması sonrası sol trigeminal alanı kapsayan hiperhidroz, hipoestezi, vestibüler hipoeksitabilite, termoregülasyon ve vazomotor bozukluk bulguları göstermektedir. Pilokarpin, atropin ve asetilkolin testleri ile terleme yanıtı objektif olarak değerlendirilmiş; bulgular, pons düzeyinde trigeminal duyu çekirdekleri ile yüksek intrasebral vejetatif merkezleri etkileyen irritatif bir lezyona işaret etmektedir.
