Abstract
Acute intermittent porphyria is a rare metabolic disorder that mimics various acute abdominal syndromes and neuropsychiatric conditions. It typically manifests between ages 20–40, more commonly in women, and presents with episodic abdominal pain, constipation, tachycardia, hypertension, peripheral neuropathy, and psychiatric symptoms. Diagnosis is confirmed by detecting porphobilinogen in urine. A 32-year-old female patient with recurrent attacks over seven years showed gastrointestinal, urinary, and neuropsychiatric symptoms. She responded positively to treatment with calcium EDTA, a chelating agent. The case highlights the diagnostic challenges and therapeutic potential of chelation therapy in acute porphyria.

Abstract
Akut intermittent porphyria, çeşitli akut karın sendromları ve nöro-psikiyatrik semptomlarla karışabilen nadir bir metabolizma hastalığıdır. Genellikle 20–40 yaş arasında ortaya çıkar ve kadınlarda daha sık görülür. Ataklar halinde seyreder; karın ağrısı, konstipasyon, taşikardi, hipertansiyon, periferik nöropati ve psikiyatrik belirtilerle kendini gösterir. Tanı, idrarda porphobilinogen saptanmasıyla konur. Yedi yıl boyunca üç atak geçiren 32 yaşındaki kadın hastada gastrointestinal, üriner ve nöro-psikiyatrik semptomlar gözlenmiş; calcium EDTA ile yapılan tedaviye olumlu yanıt alınmıştır. Bu vaka, akut porphyria’nın ayırıcı tanıdaki önemini ve şelasyon tedavisinin potansiyelini vurgulamaktadır.
