Tüberkülos Spondilitisinden İleri Gelen Parapleginin Laminektımi Ameliyatıyla Tedavisi

Abstract

Bu çalışma, tüberküloz spondiliti (Pott Hastalığı) sonucu gelişen tam parapleji (iki bacak felci) tablosunun cerrahi tedavisini incelemektedir. Makale, bacaklarında ani başlayan uyuşukluk ve ardından tam hareket kaybı gelişen 21 yaşındaki bir erkek hastanın vaka sunumuyla başlar. Yapılan tetkiklerde ve miyelografide, 2. dorsal vertebra seviyesinde omurilikte tam bir tıkanma (stop) tespit edilmiştir. Prof. Dr. Melchior tarafından gerçekleştirilen laminektomi ameliyatı sırasında, omuriliği sıkıştıran kazeifiye (peynirleşmiş) nekrotik dokular ve bir soğuk apse boşaltılmıştır. Ameliyat sonrası hastanın duyuları hızla geri gelmiş ve dokuz ay içinde bastonsuz yürüyebilir hale gelmiştir. Makale ayrıca tüberküloz spondilitinin patolojik anatomisini, soğuk apselerin omurilik üzerindeki mekanik baskısını ve laminektominin erken dönemdeki hayati önemini literatür eşliğinde tartışmaktadır.

Treatment of Paraplegia Resulting from Tuberculous Spondylitis via Laminectomy Surgery

Abstract

This paper details a successful surgical intervention for a 21-year-old male patient suffering from complete paraplegia due to tuberculous spondylitis. The patient presented with total loss of sensation and voluntary movement in the lower limbs, alongside sphincter dysfunction. Myelography revealed a complete blockage at the level of the 2nd dorsal vertebra.A laminectomy was performed by Prof. Dr. Melchior, during which a cold abscess and cheesy (caseous) necrotic tissue were found compressing the spinal cord. Histological examination confirmed typical tuberculosis foci. Post-operatively, the patient showed remarkable recovery; sensation returned within days, and within nine months, he was able to walk without assistance, regaining full control of bodily functions. The study concludes that early surgical decompression through laminectomy, combined with immobilization, offers a high chance of recovery for paraplegia caused by extradural tuberculous abscesses.
