Akut Miyeloid Lösemili Hastaların Pekiștirme Tedavisinde Hematopoetik Büyüme Faktörü Kullanımının Maliyet Etkinliği
Abstract
Amaç: Akut miyeloid lösemi (AML) kemik iliğinin habis ve klonal bir hastalığı olup hematopoetik
öncül hücrelerin neoplastik dönüșümü ile gelișir. Bu çalıșma; AML tedavisinde kullanılan G-CSF’ün
nötropeni süresi, enfeksiyon sıklığı, febril nötropeni süresi, antibiyotik ve/veya antifungal kullanım
süresi ve maliyet üzerine etkisini araștırmak üzere planlandı.
Yöntemler: Çalıșmaya toplam 40 hasta alındı (19 kadın, 21 erkek, yaș ortalaması 42). Hastalar G-CSF
kullanılan ve kullanılmayan iki gruba ayrıldı. Nötropeni gelișme zamanı, nötropeni süresi, ateșin
ortaya çıkıș zamanı, ateșin süresi, kültürle ispatlanan enfeksiyon, antibiyotik ve antifungal kullanım
süresi, hastanede yatıș süresi, antibiyoterapi maliyeti ve toplam maliyet verileri iki grup arasında
kıyaslandı.
Bulgular: G-CSF grubunda; nötropeni süresi, ateș ortaya çıkıș zamanı, hastane yatıș süresi, ateș süresi,
antibiyotik ve antifungal tedavi süresi ve antibiyoterapi maliyeti azaldı. G-CSF grubunda;
antibiyoterapi dıșı günlük hastane maliyeti ve toplam maliyette hafif azalma mevcuttu (P: 0,751).
Sonuç: AML’li hastaların pekiștirme tedavisinde kullanılan yüksek doz ARA-C protokolüne G-CSF
grubunda eklenmesi nötropeni ve hastanede yatıș süresini anlamlı olarak kısaltır (p
