Abstract

1967-1983 yılları arasında Ankara Üniversitesi Tıp Fakültesi Uroloji Kliniğinde toplam 241 hastaya netrostomi yapılmıştır. Nefrostominin en sık taş cerrahisinde, 178 olguda (6 73,9) uygulandığı görülmüştür.'24 pyonefroz (“2 10) ve 15 pyeloplasti (9: 6,3) bunu takip etmwsktedir. Maligniteye bağlı bilateral ureter obstrüksiyonu olan 5 olgüda (X 2,1) nefrostomi yapılması zorunlu olmuştur. 6 anüri kalküloz ve ? histerektomi sonucu bilateral ureter ligasyonuna bağlı anüri “sunucu acil nefrostomi yapılmıştır.

Nefrostomi endikasyonları halen tartışmalı olup özellikle acil durumlarda oldukça riskli bir işlem olarak kabul edilmektedir. Günümüzde açık operasyonların kontrendike olduğu hallerde klasik nefrostominin yerini perkütan nefrostomi almaya başlamıştır. Uygulamasının kolaylığı, operasyon işlemine gerek kalmaması, komplikasyon
oranının düşük oluşu ve drenaj sağlanır sağlanmaz fonksiyonal ve radyolojik araştırmalara başlanabilmesi nedeni ile, medikal taşe'haslıkları tedavisindeki güçlüklere karşı olarak bugün yaygın olarak kullanılan bir yöntem haline gelmiştir.

Nefrostomi halen, hayat kurtarıcı bir işlem olarak, uro'oji ala nında güncelliğini devam ettiren önemli bir operasyon yöntemi olarak kabul edilmektedir.

Indications for Nephrostomy

Abstract

In the Urology Department of Ankara Medical Faculty, 241 patients have undergone nephrostomy between 1967-1983.Nephrostomy was most İreguentiy performed on 178 cases (76 73,9) with urinary stones, followed by 24 cases of pyonephrosis (7 10) and 15 cases of pyeloplasty. 5 cases with bilateral ureteral obstruction due to malignancy (X 2.1) have undergone obligatory nephrostomy. 6 cases of anuria due to urinary stones and 2 cases of iatrogenic ureteral ligation after hysterectomy have undergone emergent, nephrostomy. Also nephrostomy was performed on 7 cases ureterovesical obstruction and then they were undergone definitive surgical
correction. Indications of nephrostomy are still on debate, and especialiy in emergenit cases, it İS accepted as a procedure of high risk. Recentiy, in cases of which open surgical procedures are thought to be contraindicated, percutaneus nephrostomy has taken place of formal nephrostomy. It is easy to perform and there is no need for an operative, procedure, and the incidenceof its complications istoo low, and when drainage is provided the functional and radiologic investigations can easly be performed within a short time. For all these reasons, it has been a popular procedure nowadays.In Urology, nephrostomy,as& life saving procedure, is an importent surgical method and we believe that it's popularity will always survive.
