Sol Ventrikül Volüm Yüklenmesinin Elektrokardiografik Kriterlerinin Anjiokardiografik Ve Ekokardiografik Bulgularla Karşılaştırılması

Abstract

A.Ü. Tıp Fakültesi kardiyoloji kliniğinde yatan saf aort yetmezliği olan 17 olgunun sol ventrikül fonksiyonları elektrokardiografik, ekokardiografik ve anjiokardiografik olarak incelendi. I - EKG deki sol ventrikül voltaj kriterleri (SVı - RV: ve SV; - RVs ile ekokardiografideki sistol sonu ve diastol sonu çap, sol ventrikülün kitlesi, sistol sonu ve diastol sonu volümler arasındaki ilişki anlamlı, İVS ile EKG deki SV, -- RVs ile az anlamlı bir ilişki (p<0.05), İVS ile EKG deki SV, - RV: arasındaki ilişki anlamsız, bulunmuştur. EKG deki her iki voltaj kriteri ile ekokardiografideki sol venirikül arkaduvar kalınlığı arasındaki ilişki anlamsız bulunmuştur. ORS aksisi ile ekokardiografideki sistol sonu ve diastol sonu volüm sol ventrikül kitlesi, ejkesiyon fraksiyonu, sirkumferensiyal fibril kısalma, hızı, so İventrikül arka duvar kalınlığı, sistol sonu ve diastol sonu çap, atım volümü ve ivs kalınlığı arasındaki ilişki istatistiki bakımdan anlamsız bulunmuştur. 2 — EKG deki her iki voltaj kriteri ile anjiokardiografideki sistol sonu ve diastol sonu volümler arasındaki ilişki anlamlı bulunmuştur. EKG deki ORS aksisi ile anjiokardiografiden hesaplanan sistol sonu, diastol sonu volüm v eejeksiyon fraksiyonu arasındaki ilişki anlamsız bulunmuştur. 3 — Ekokardiografik ve anjiokardiografik olarak ölçülen ejeksiyon fraksiyonu, sistol sonu ve diastol sonu volümler arasındaki ilişki oldukça anlamlı bulunmuştur. Sonuç olarak aort yetmezliğinde hasta için az da olsa sakıncası bulunan ve pahalı bir yöntem olan hemodinamik çalışma yapmadan tekrarda sakıncası olmayan, uygulama güçlüğü göstermeyen bir yöntem olan ekokardiografi ile sol ventrikül fonksiyonlarını değerlendirebiliriz.

Comparison of Electrocardiographic Criteria for Left Ventricular Volume Loading with Angiocardiographic and Echocardiographic Findings

Abstract

The left ventricular function of 17 patients who were hospitalized at the department of Cardiology, the Faculty of Medicine of Ankara University wtih pure aortic regürgitation was studied electrocardiographycally and angiocardiographicalIy. I - The relation between the left ventricular voltage criteria in ECG (SV.--RV: and SV.4--RV) and the end-systolic and end-diastolic diameters, the mass of the left ventricle, the end-systolic and end-diastolic volumes was found to be significant while the relation between the posterior wall thickness of the left ventricle and the voltage criteria in ECG was insignificant. The relation between İVS and SV; RV: in ECG was found to be significant, whereas the relation between İVS and SV: - RV: was insignificant. The relation between th e axis of ORS and the end-systolic and end-Diastolic volüme iri echocardiography, the mass of the left ventricle the ejection fraction, the rate of sircumferantiol fiber shortening, the posterior wall thickness of the left ventricle, the end-systolic and end-diastolic diameter, the stroke volüme and the thickness of the İVS was found to be statishcally insignificant. o 2 — The relation between both of the voltage ciriteria in ECG and the end-sistolic and end-diastolic volumes, the ejection fraction was found to be significant. The relation between the axis of ORS in ECG and the end-sistolic and end-diastolic volumes, the ejection fraction calculated from angiocardiographic data was fount to be insignificant. 3 — The relation between ejection İraction and the end-sistolic and end-diastolic volumes measured echocardiographically and angio cardiographically was found to be significant. İn conclucion, wecan evaluate the left ventricular function by echocardiography a practically available method which has no risks with no need for hemodynamic study, an expensive and risky method for a patient with aortiç regürgitation.
