Lieno - Renal Shunt in Surgical Treatment of Renovascular Hypertension

Abstract

Son on yılda, tanı ve tedavi yöntemlerindeki gelişmeler, renovasküler hipertansiyonun tedavisini olumlu yönde ve büyük çapte etkilemiştir. Başarıdaki önemli faktörlerden biriside, rekonstruktif damar cerrahisindeki gelişmeler yanında, ayrıntılı diagnostik araştırmalar sonucu, revaskülarizasyon yapılacak olan hastaların iyi seçilmesidir (2). Renovasküler hipertansiyonun cerrahi tedavisinde, çeşitli ameliyat yöntemleri uygulanmıştır (9). İlk zamanlar uygulanan nefrektomi ameliyatı, bu gün en son düşünülecek tedavi yöntemidir. Bu görüş, özellikle iki taraflı oluşan ve progresif seyredebilen fibromüsküler displazi vak'alarında daha geçerlidir. Bugün genellikle, aorta-renal By-Pass (şant) en çok tercih edilen yöntemdir (3).
Sentetik greftler, otojen arter ve otojen saten veni, renal arterin rekonstrüksiyonunda başarılı olarak kullanılmıştır. Bu greftlerden vena Safena, kolay elde edilmesi ve otojen oluşu nedeniyle, bugün genellikle yaygınlık kazanmıştır (6,8,15).
Bu yazıda, sol renal arterde, fibromüsküler displaziye bağlı, renovasküler hipertansiyonu olan genç bir hastada, izah edilen görüşlerin dışında, pek uygulanmayan A. Lienalis ile yapılan başarılı bir şant tarif edilmiştir.

Renovasküler Hipertansiyonun Cerrahi Tedavisinde Lieno - Renal Şant

Abstract

In the last decade, developments in diagnostic and therapeutic methods have positively and significantly affected the treatment of renovascular hypertension. One of the important factors in success is the developments in reconstructive vascular surgery, as well as the careful selection of patients who will undergo revascularization as a result of detailed diagnostic studies (2). Various surgical methods have been applied in the surgical treatment of renovascular hypertension (9). Nephrectomy surgery, which was applied in the early days, is today the last treatment method to be considered. This view is especially valid in cases of bilateral and progressive fibromuscular dysplasia. Today, aorta-renal bypass (shunt) is generally the most preferred method (3).
Synthetic grafts, autogenous artery and autogenous satin vein have been successfully used in the reconstruction of the renal artery. Of these grafts, the saphenous vein has become widespread today due to its easy obtainability and autogenous nature (6,8,15).
In this article, a successful shunt in the left renal artery, performed with A. Lienalis, which is rarely used, outside of the explained views, in a young patient with renovascular hypertension due to fibromuscular dysplasia, is described.
