Abstract

This report presents a rare case of renal cholesteatoma accompanied by ureter and bladder stones in a 54-year-old male, marking the first such instance recorded in Turkey and the 14th globally. The patient exhibited a prolonged history of intermittent flank pain, recurrent urinary infections, and pyuria spanning nearly a decade. Surgical intervention included multiple procedures culminating in nephrectomy. Histopathological examination revealed squamous metaplasia with keratin-rich globular formations infiltrating the renal parenchyma, without atypia or malignancy. The discussion compares findings with previous literature and emphasizes the importance of considering cholesteatoma in chronic urinary tract infections with persistent keratinous debris.

A Kidney Cholesteatoma Associated with Ureter and Bladder Stone

Abstract

Bu çalışmada, 54 yaşında erkek bir hastada sol böbrek kolesteatomu ile birlikte sol üreter ve mesanede taş görülmesi sunulmuştur. Bu vaka, Türkiye’de bildirilen ilk ve dünya literatüründeki 14. renal kolesteatoma vakasıdır. Hastada yaklaşık 10 yıl boyunca devam eden aralıklı bel ağrısı, tekrarlayan üriner enfeksiyonlar ve pyüri mevcuttu. Yapılan cerrahi müdahaleler sonrası sol böbrek nefrektomisi uygulanmıştır. Histopatolojik incelemede squamous metaplazi, lameller keratin kitleleri ve glop korne benzeri oluşumlar saptanmış; atipi ve malignite bulgusuna rastlanmamıştır. Literatürle karşılaştırmalı değerlendirme yapılmış, uzun süreli pyüri ve keratinöz materyal varlığında kolesteatoma olasılığına dikkat çekilmiştir.
