Abstract

The paper discusses the principles, rationale, and technical details of bilateral selective gastric vagotomy as a surgical method for treating peptic ulcers. It compares vagotomy methods (with or without drainage) and surgical combinations such as antrectomy and gastric resection, assessing their respective indications and complications. Selection of technique is based on clinical presentation, ulcer location, and acid secretion profiles. The article details operative criteria and describes both anatomical and skeletalization-based approaches to vagal nerve dissection. It emphasizes careful nerve identification, the role of vagotomy completeness, and the use of post-operative functional tests. Several pyloroplasty techniques (e.g., Heineke-Mikulicz, Finney) are illustrated. The report concludes that individualized decision-making based on ulcer characteristics is essential, and that complete vagotomy and properly selected drainage procedures significantly influence outcomes.

Çift Taraflı Selektif Gastrik Vagotomi — Endikasyonları, Kontrendikasyonları ve Ameliyat Tekniği

Abstract

Bu makalede peptik ülser tedavisinde cerrahi yaklaşım olarak çift taraflı selektif gastrik vagotominin prensipleri, endikasyonları ve cerrahi teknik detayları ele alınmıştır. Vagotomi çeşitleri (drenajlı veya drenajsız) ve antrektomi, rezeksiyon gibi kombine ameliyatlar karşılaştırılarak, klinik tablo, ülserin lokalizasyonu ve mide asit sekresyonu gibi kriterlere göre teknik seçimi tartışılmıştır. Anatomik ve skeletizasyon metodlarıyla vagus sinirlerinin disseksiyonu ayrıntılı biçimde aktarılmış; vagotominin eksiksiz yapılmasının önemi vurgulanmış ve postoperatif fonksiyonel testlere yer verilmiştir. Ayrıca çeşitli piloroplasti teknikleri (Heineke-Mikulicz, Finney vb.) şekillerle sunulmuştur. Sonuç olarak, ülserin tipine göre bireyselleştirilmiş yaklaşımın gerekli olduğu, tam vagotomi ve uygun drenaj ameliyatının sonuçlar üzerinde belirleyici etkisi olduğu belirtilmiştir.
