Diffüz Malign Plevra Mezotelyomaları Üzerine Klinik Araştırma

Abstract

This study reviews the clinical findings of 17 cases of diffuse malignant pleural mesothelioma diagnosed over the last decade at the Thoracic and Cardiac Surgery Department of Ankara University Medical Faculty. The diagnosis was predominantly made through surgical specimens obtained during thoracotomy procedures. Histopathological analyses revealed various subtypes, including papillary, epithelial, and carcinosarcoma forms. Diffuse spread was present in the majority of cases, with only one displaying a localized mesothelioma pattern. The average patient age was 43.4, and no distinct gender prevalence was identified. Due to limitations in anamnesis and laboratory methods, exposure to asbestos could not be confirmed, though global literature was reviewed for comparison on environmental factors. The importance of intraoperative macroscopic evaluation in diagnosis was emphasized. Treatment modalities included cytostatic agents, radiotherapy, and radioactive interventions.

Clinical Investigation of Diffuse Malignant Pleural Mesothelioma

Abstract

Bu çalışma, Ankara Üniversitesi Tıp Fakültesi Göğüs ve Kalp Cerrahisi Kliniği’nde son 10 yıl içinde tespit edilen 17 diffüz malign plevra mezotelyoma vakasının klinik bulgularını değerlendirmektedir. Hastalık, genellikle cerrahi müdahale sırasında elde edilen doku örnekleriyle teşhis edilmiştir. Histopatolojik incelemeler sonucunda vakaların farklı alt tipleri belirlenmiş; papiller, epitelyal ve karsinosarkom gibi. Vakaların çoğunda diffüz yayılım gözlenmiş, yalnızca birinde lokalize mezotelyoma bulgusu saptanmıştır. Hastaların yaş ortalaması 43.4 olup cinsiyet dağılımında belirgin fark bulunmamaktadır. Asbeste maruz kalma, anamnez ve laboratuvar eksiklikleri nedeniyle doğrulanamamış; ancak endüstriyel ve çevresel etkenlerin rolüne dair uluslararası literatürle karşılaştırmalı değerlendirme yapılmıştır. Hastalığın tanısında torakotomi bulgularının önemi vurgulanmış, tedavi seçenekleri arasında sitostatik ilaçlar, radyoterapi ve radyoaktif ajanlar yer almıştır.
