Abstract

This article discusses a rare case of traumatic vulvar hematoma observed at the Department of Obstetrics and Gynecology, Ankara University. The patient, a young unmarried woman, sustained injury during a bathroom accident, resulting in a rapidly enlarging hematoma that fully obstructed the vulvar introitus. Given the progressive pain and increasing size of the hematoma, surgical intervention was performed to evacuate the hematoma and achieve hemostasis. The article explores the anatomical predisposition, traumatic and obstetric causes, clinical manifestations, differential diagnoses, complications, and therapeutic approaches to vulvar and vaginal hematomas. Prompt diagnosis and intervention are emphasized as key to favorable outcomes.

Abstract

Bu makalede, Ankara Tıp Fakültesi Kadın Hastalıkları ve Doğum Kliniği’nde gözlemlenen nadir bir travmatik vulva hematomu vakası üzerinden vulva-vagina hematomlarının klinik özellikleri ve tedavi yaklaşımları değerlendirilmiştir. Evlenmemiş genç bir kadında, kazara bacaklarının açık pozisyonda bir teneke üzerine düşmesi sonucu gelişen hematom, hızla büyüyerek introitusu tamamen örtmüştür. Artan ağrı ve hematom hacmi nedeniyle konservatif tedavi yeterli görülmemiş ve operatif müdahale ile pıhtı boşaltılıp kanayan damarlar kontrol altına alınmıştır. Literatür ışığında, vulva-vagina hematomlarının etiyolojisi, anatomik yatkınlık, travmatik ve obstetrik nedenler, klinik belirtiler, ayırıcı tanı, komplikasyonlar ve tedavi seçenekleri detaylı olarak sunulmuştur. Erken müdahale ile prognozun olumlu yönde değişebileceği vurgulanmıştır.
