İndirekt Porto-Kaval Şant İçin Dakron Greft Kullanılması (V. cava inf. ve V. mesent. sup. arası şant)

Abstract

Portal hipertansiyon nedeniyle özofagus varis kanaması olan iki olguda, arteriyel Dakron greft ile şant operasyonu yapıldı. Bunlardan birinde spleno-renal Terminal anastomozu Dakron greft ile yapıldı. Bir diğer olguda ise portal ven trombozu mevcuttu. Bu nedenle portokaval şant yapmak mümkün olmadı. Sup. mes. veni ile şant yapmayı denedik, ancak bu damarda da 2x2 cm'lik tromboz vardı. Damar açıldı ve tromboz çıkarıldı. Bu işlemden sonra yüksek basınçlı venöz kanama başladı. Sup. mes. veni ile vena kava arasında Dakron greft ile şant yapmaya karar verdik. 1 cm çapında greft kullandık ve latero-lateral vena kava ile sup. mes. ven anastomozu yaptık. İki hasta iyi tolere edildi ve iyi durumda taburcu edildi. İki hastayı altı ay boyunca takip ettik. Ayrıca durumları da iyiydi.

Use of Dacron Graft for Indirect Porto-Caval Shunt (Shunt between vena cava inf. and vena mesent. sup.)

Abstract

In the two cases of esophageal varices bleeding because of
portal hypertension, shunt operation has been done by means of
arterial Dacron graft. In one of those spleno-renal Termino Terminal
anastomosis has been done through Dacron graft. In another case
there was portal vein Thrombosis. For this reason it was impossible
to do porto-caval shunt. We tried to do shunt by means of sup. mes.
vein, but there Was also 2x2 cm. Thrombosis in this vein. The vein
was opened up and the Thrombosis was extracted. After this procedure
the venous bleeding started with high pressure. We decided to
make a shunt between the sup. mes. vein and the vena cava through
the Dacron graft.
We used I cm. diameter graft and made latero-lateral vena cava
and sup. mes. vein anastomosis. Two patients tolerated well and
discharged in a good condition, We followed up the two patients for
six months. They were also in a good condition.
