Böbrek Taş Hastalığının Cerrahi Tedavisinde Avasküler Hipotermi Yönteminin Değeri

Abstract

Bu çalışmada, böbrek taşlarının cerrahi tedavisinde böbrek dokusunu koruma ilkesine dayanan avasküler hipotermi yönteminin etkinliği değerlendirilmiştir. A.Ü. Tıp Fakültesi Üroloji Kliniği’nde 1979–1982 yılları arasında böbreğinde çok sayıda taş bulunan 30 olguya iki farklı renal hipotermi tekniği uygulanmıştır: 10 olguya Blandy'nin basit renal hipotermisi, 20 olguya ise Wickham'ın yöntemi esas alınarak geliştirilen modifiye renal hipotermi ünitesi ile anatrofik nefrolitotomi yapılmıştır. Radial nefrotomiler sayesinde vasküler yapılara zarar vermeden taşlara ulaşılmış; postoperatif komplikasyonlar ve kan transfüzyonu ihtiyacı minimal düzeyde kalmıştır. Hastaların büyük kısmı 7–12 gün içinde taburcu olmuş; rezidüel taş oranı düşük bulunmuştur. Çalışma, modifiye renal hipotermi ünitesinin hem cerrahi kontrolü kolaylaştırması hem de böbrek fonksiyonlarını koruyucu etkisi sayesinde konservatif böbrek cerrahisinde tercih edilebilir bir yaklaşım olduğunu göstermektedir.

The Value of Avascular Hypothermia Method in the Surgical Treatment of Renal Calculi

Abstract

This study evaluates the efficacy of avascular hypothermia techniques in preserving renal parenchyma during surgical treatment of kidney stones. Conducted at Ankara University Faculty of Medicine’s Urology Clinic between 1979–1982, 30 cases with multiple renal calculi underwent anatrophic nephrolithotomy via two approaches: Blandy’s simple hypothermia (10 cases) and a modified Wickham hypothermia unit developed locally (20 cases). Radial nephrotomies allowed stone removal without damaging vascular structures, resulting in minimal postoperative complications and limited need for blood transfusion. Most patients were discharged within 7–12 days, and residual stone rates were low. The findings suggest that the modified hypothermia unit facilitates surgical control and effectively preserves renal function, making it a favorable conservative strategy in stone surgery.
