Clinical Pharmacology of Senile Dementia

Abstract

This review analyzes the efficacy and limitations of pharmacological agents used in the treatment of senile dementia. Methodological flaws in clinical studies—including small sample sizes, heterogeneous patient populations, short trial durations, and lack of placebo control—have complicated the interpretation of drug effectiveness. Most vasodilators, nootropic agents, neurotransmitter precursors, and neuropeptides have shown no significant benefit beyond placebo. However, compounds such as hydergine and vasopressin have demonstrated limited therapeutic potential. Advances in neurochemical research related to Alzheimer’s disease and dementia offer promising avenues for developing more effective treatments.

Senil Demansın Klinik Farmakolojisi

Abstract

Bu derleme, senil demansın tedavisinde kullanılan farmakolojik ajanların etkinliğini ve klinik kullanımdaki sınırlılıklarını ele almaktadır. Literatürdeki çalışmalarda yer alan metodolojik hatalar — düşük hasta sayısı, heterojen vaka grupları, kısa süreli denemeler ve plasebo kontrol eksiklikleri — ilaca dayalı tedavilerin yorumlanmasını güçleştirmektedir. Vazodilatörler, nootropik ajanlar, nörotransmitter öncüleri ve nöropeptidlerin birçoğunun plasebodan üstün olmadığı saptanmış; yalnızca hiderjin ve vazopressin gibi bazı bileşikler sınırlı fayda göstermiştir. Alzheimer hastalığı ve demansla ilişkili nörokimyasal bozukluklar üzerine yapılan araştırmalar, etkili tedavi geliştirme yolunda umut vermektedir.
