Renal Parenchymal Cancer

Abstract

Enstitümüze 1947 - 1971 yılları arasında gelen biopsi ve otopsi materyeli böbrek parankim kanseri yönünden i ncelendi. Histogenez ve nomenklatürü tartışıldı. Vakaların makroskopik görünüşü, varyasyonları gözden geçirildi ve yeni bir derecelendirme sistemi ortaya kondu. Derecelendirme bilhassa hücre tipi, fonksiyon kaybı, yağ ve glikojen absorbsiyonu tipi ve differansiyasyona göre yapıldı. Tümörün hacmi, nekroz, kalsifikasyon, hemoraji ve psödo kist yapıları histolojik derecelendirmede önemli bulunmadı.
Derece I : Sadece şeffaf hücreler ihtiva eder veya böbrekteki tubulileri hatırlatan diferansiye granüler hücreler bulunmaktadır.
Derece II. Şeffaf hücre ve küçük granüler hücrelerin bir karışımına sahiptir veya tubuler ve trabeküler yapılar ihtiva eder ve küçük füziform hücre grupları vardır.
Derece III : Herhangi bir organizasyon göstermeyen küçük granüler hücrelerle fuziform hücrelerden ibarettir. indiferan hücre grupları, birkaç dev hücresi olabilir.
Derece VI : indifferan olan tümörlerdir. Bunlar Liposarkomu Fibrosarkomu hatırlatabilir.
63 vaka ve 31 mikrofotograf taktim edildi. Serinin nadir vakaları münakaşa edildi ve literatür gözden geçrildi.

Abstract

Carcinoma of renal parenfhyma on both surgical and autopsy material subm.itted to the Department of Pathology, University
of Ankara Medical School, between the years 1947 - 1971 have been studied. Nomenclature and histcgenesis are briefly discussed. Gross appearence and variations have been evaluated and a new grading system is proposed. The system is based mainly on cell type, the degree loss of function - Such as absorbtion of fat and glycogen and differantiation. The size of tumour, necrosis, calcification, hemorrhage and pseudocyst formation are found to being unimportant as far as histological grading is concerned.
Grade I : Turnours consist only of clear cells or differentiated granular cells resembling tubular cells of kidney with tubular structures. Grade : Thmours made up of mixture of clear cells and small granular cells or small patches of fusiform cells with gome tubular, trabecular structures. Grade Ill: tLümours of small granular cell or fusiform cells without any structural pattern. There may be some islands of differentiated cells, clear cells and few bizarre cells Grade IV : Tumours of undifferentiated type have been put in grade IV. Turnours resembling liposarcoma, fibrosarcoma with many tumor giant cells are fin this grade which are difficult to diagnose at metastatic site or in small biopsies from «the retroperitoneal mass»
