Technical Features of the Modified Harkins Procedure in Duodenal Ulcer Cases

Abstract

This technical paper elaborates on Prof. Bumin’s modified version of the Harkins procedure, combining selective gastric vagotomy, hemigastrectomy, and gastroduodenostomy. Key innovations include performing selective vagotomy post-duodenal resection to allow full visualization of stomach innervation, single-layer silk suturing of the lesser curvature for better tissue healing, and stabilization of the stomach’s posterior wall onto the pancreatic head serosa. The author highlights benefits such as reduced necrosis risk from anomalous hepatic artery ligation and avoidance of dumping syndrome through optimized anastomotic positioning. A specialized parallel incision technique on the duodenum, aligning with vasa recta, is proposed to enhance anastomotic integrity and perfusion. These modifications aim to maintain physiological reservoir function, minimize postoperative complications, and avoid recurrence.

Duodenum Ülserlerinde Değiştirerek Uyguladığımız Harkins Ameliyatının Teknik Özellikleri

Abstract

Bu teknik yazıda Prof. Dr. Bumin'in modifiye Harkins yöntemi kapsamındaki seçici gastrik vagotomi, hemigastrektomi ve gastroduodenostomi uygulamasının detayları aktarılmaktadır. Vagotomi'nin, duodenum kesildikten sonra uygulanması sayesinde mide sinir dallarının daha net görülmesi sağlanmıştır. Küçük kurvatür, içten bir sıra tek tek ipek dikişle kapatılmış; böylece nedbe dokusu minimal tutulmuş ve mide işlevleri erken dönemde normale dönmüştür. Gastroduodenostomi bölgesi, pankreas başı üzerine sabitlenerek sızıntı riski azaltılmış, dumping sendromu önlenmiştir. Ayrıca duodenumda vasa recta'lara paralel kesim önerilerek dış dudak beslenmesi artırılmış ve anatomik uyum desteklenmiştir. Tüm bu teknik geliştirmelerle mide rezervuar işlevi korunmuş, komplikasyonlar azaltılmış ve nüks riski en aza indirilmiştir.
