Türkiye’de Endemik Guvatr 3 — İyod Yetersizliğinin İyod Metabolizması Üzerindeki Akisleri

Abstract

İyod yetersizliğine adaptasyon mekanizmalarının incelenmesile memleketimizdeki endemik guvatr vak'alarının etiyolojisinde iyod yetersizliğinin rolünü tesbit etmek kabilidir. Filhakika vak'alarımızın mühim bir kısmında 4 ve 24 saatlik I¹³¹ «uptake» inin yüksek bulunması, PBI¹³¹ in % 16 nisbetinde tirotoksikoz seviyelerinde bulunması, plazma inorganik iyodu ve 24 saatte idrarla itrah edilen iyodun düşüklüğü memleketimizde iyod yetersizliğinin mevcudiyetini ispat etmektedir. PBI seviyelerle, günlük hormon istihsali seviyelerinin normal bulunması ise bu iyod yetersizliğinin diğer bazı memleketlerdeki kadar şiddetli olmadığını meydana çıkarmaktadır. Diğer taraftan bu inceleme sonucunda I¹³¹ «uptake» lerinin, PBI¹³¹ ve PBI un normal bulunduğu endemik guvatr vak'alarında tiroid bezinin hiperplazisinin direkt olarak iyod yetersizliğine bağlı bir adaptasyon faktörü olduğu kanısına varılmış ve bu mesele münakaşa edilmiştir.

Endemic Goiter in Turkey 3 — Iodine metabolism in iodine deficiency

Abstract

The study of the adaptation mechanisms to iodine deficiency can enable us to determine the etiologic role of iodine deficiency in the production of endemic goiter in Turkey. Indeed, the high I¹³¹ uptake by thyroid gland in a very large portion of our endemic goiter cases, the high PBI¹³¹ in 16 % of our patients, the low plasma inorganic iodine and 24 hr. urinary iodine excretion levels found in the same patients are all in favor of existence of an iodine deficiency in the endemic goiter regions of Turkey. But, the normal levels of PBI and of hormonal release prove that this iodine deficiency is not so severe as in Mendoza, Mulia, Congo etc..

We also believe and discuss that the thyroid hyperlasia due to iodine deficiency in endemic goiter cases with normal I¹³¹ thyroid uptake, PBI¹³¹ and PBI, is an adaptation factor directly related to iodine deficiency.
