Leiomyosarcoma is a rare malignant tumor originating from smooth muscle, and even rarer when located in the duodenum. This case details a 55-year-old man admitted with gastrointestinal bleeding, anemia, and signs of duodenal ulceration. Radiological and clinical evaluations suggested a duodenal mass, and surgery revealed a 3 cm ulcerated tumor in the third part of the duodenum. Pathological analysis confirmed leiomyosarcoma. Despite initial postoperative recovery, metastases were found in the liver and lungs within three months, and the patient passed away. This report includes a comparative review of 63 previously published cases, discussing typical locations, symptoms, radiological features, and histological challenges of diagnosis.
