Organic and Functional Tricuspid Regurgitation

Abstract

The clinical and hemodynamic findings of 18 cases, in which functional or organic tricuspid insufficiency has been diagnosed during the operation have been reviewed.
in 9 cases diagnosed as organic tricuspid lesion the clinical signs were more pronounced, the c wave had not formed and the x through had disappeared.
in 9 cases diagnosed as functional tricuspid insuffiency, the c wave had formed and the x through was present.
In both, the organic and functional tricuspid insufficiencies it has been found that the mean pressure of right atrium, right vetricle, pulmonary artery, and the pulmonary capillaries are of no diagnostic value.
From the right atrial pressure curves of 10 normal cases and functional tricuspid insufficiency, rate of rise of v wave, peak time of the v wave and modified pressure factor has been calculated and the avarages of these findings have been compared.
In relation to the normal cases the rate of rice of the v wave has increased in both types of tricuspid insuffiencies.
It was noticed that the peak time of the v wave was a little late in the organics than the functionals, in boh it forms second earlier than the normals.
It was concluded that the pressure factor ise of no diagnostic value.

Organik ve Fonksiyonel Triküspit Yetersizlikleri

Abstract

Ameliyatla fonksiyonel veya organik triküspid yetmezliği tesbit edilen 18 vak'anın klinik ve hemodinamik bulguları incelendi.
Organik triküspid lezyon tanısı alan 9 vak'ada; triküspid yetmezliğine bağlı klinik belirtiler daha şiddetli idi; c dalgası teşekkül etmemiş ve x çöküntüsü kaybolmuştu.
Fonksiyonel triküspid tanısı alan 9 vak'ada; c dalgası teşekkül etmiş, x çöküntüsü kaybolmamıştır.
Gerek organik gerek fonksiyonel yetmezliklerde sağ atrium sağ ventıikül, pulmoner kapiller basınçlarmn ortalamalarmn diağnostik bir değeri olmadığı bulundu.
Normal 10 vak'a ile organik ve fonksiyonel triküspid yetmezliği vak'alarında sağ atrium basınç eğrisinden v çıkış hızı, v zirve zamanı ve modifiye basınç faktörü hesaplandı. Bulunan değerlerin ortalamaları karşılaştırıldı. Normal vak'alara nazaran her iki tip triküspid yetmezliklerinde v çıkış hızı artmıştır; v zirve zamanınınm organiklerde fonksiyonellere nazaran biraz daha geç teşekkül ettiği, normallere nazaran ise her iki tip triküspid yetmezliğinde, 0,07-0,09 saniye erken oluştuğu görüldü.
Basınç faktörünün diagnositik bir değeri' olmadığı kabul edildi
