Abstract

Bronchus stump fistulas seen after pneumonectomies are a serious complication that is not uncommon. In cases where tuberculosis is not present in the etiology, this serious complication should always be kept in mind. In cases where the stump is opened, the surgical technique becomes very difficult in closing the opening because empyema occurs. The transpleural route is about to be abandoned due to empyema with a new approach. Intervention with mediansternotomy, as accepted by us, has been used in many clinics in recent years. However, there are still major problems with the stumfun closure technique and it is being reopened at a large rate. In our opinion, the application of pleural pericardium flaps is impossible in this case, both due to anatomical changes and infection. For this reason, we used LYODURA plastic for the first time in our clinic and in our country. The results we have obtained with the application of Homolog Dural, presented by BRAUNMELSUNGEN company, are very positive. In our opinion, it is the most suitable method for the repair of defects in thoracic surgery today.

Tüberküloz Dolayısıyla Yapılan Plöropnömonektomiden Sonra Görülen Bronchus Fistülünün Transsternal Yolla Reamputasyonu Ve Lyodura Plastiği İle Yapılan Cerrahi Tedavisi

Abstract

Bronchus stumfunun pnömenektomilerden sonra görülen fistülleri nadir olmayan ciddi bir komplikasyondur. Etyolojisinde tuberküloz mevcut olmayan vakalarda bu ciddi komplikasyon daima ön planda tutulmalıdır. Stumfun açılmış olduğu vakalarda empyem teşekkül ettiği için açığın kapatılmasında cerrahi teknik çok zorlaşır. Transpleural yol empiyem nedeni ile yeni bir akımla terkedilmek üzeredir. Mediansternotomi ile müdahale tarafımızdan da kabul edildiği gibi birçok kliniklerde son senelerde kullanılmaya başlanmıştır. Ancak stumfun kapatılma tekniğinde halâ büyük sorunlar mevcut olup büyük bir oranda tekrar açılmaktadır. Kanaatimizce plevra pericardium flepleri tatbiki bu durumda gerek anatomik değişme ve gerekse infeksiyon sebebi ile imkânsızdür. Bu sebeple ilk defa kliniğimizde ve memleketimizde LYODURA plastiği kullandık. BRAUNMELSUNGEN firmasının takdim ettiği Homolog duranın tatbiki ile aldığımız netice çok müsbettir. Kanaatimizce bugün için torasik cerrahide defektlerin tamirinde ortaya çıkan güçlükler LYODURA’nın kullanılması ile büyük nisbette ortadan kalkacaktır.
