Depresyonda Kısa Süreli Rehberli İnternet Temelli Bilişsel Davranışçı Terapi: Olgu Sunumu

Abstract

Bu makale, depresyon tedavisinde kısa süreli rehberli İnternet Temelli Bilişsel Davranışçı Terapi (iBDT) programının etkilerini bir olgu sunumu üzerinden incelemektedir. Geniş çaplı araştırmalar, iBDT’nin depresyon ve anksiyete bozuklukları üzerindeki etkinliğini desteklemekte olup, bu çalışma özellikle rehberli ve kısa süreli uygulamanın depresyon belirtileri üzerindeki etkilerini değerlendirmeye odaklanmaktadır. Çalışma kapsamında, orta şiddette depresyon belirtileri gösteren yetişkin bir kadının rehberli iBDT müdahalesine katılımı ve bu müdahalenin sonuçları ele alınmıştır. Bu olgu sunumunda kullanılan program, depresyona özgü bilişsel-davranışçı teknikleri içeren 15 çevrimiçi modülden ve bu modülleri destekleyen 6 yüz yüze seanstan oluşmaktadır. Modüllerde yer alan psikoeğitim, davranışsal aktivasyon, otomatik düşünceleri tanıma, bilişsel yeniden yapılandırma, ara inanç ve temel inançlarla çalışma gibi beceriler seanslarda terapötik bir yaklaşımla desteklenmiştir. Süreç boyunca depresyon seviyeleri Beck Depresyon Envanteri (BDE) ile izlenmiş; tedavi öncesinde 22 olarak ölçülen BDE skoru, müdahale sonunda 0'a düşmüş ve danışanın genel yaşam kalitesi ile günlük işlevselliğinde belirgin iyileşmeler gözlenmiştir. Bu olgu sunumu, rehberli iBDT'nin kısa süreli uygulamasının depresyon semptomlarını azaltmada etkili bir yöntem olabileceğini ve psikoterapiye erişimdeki mevcut kısıtlamaları aşmada değerli bir araç olarak değerlendirilebileceğini göstermektedir.

Brief Guided Internet-Based Cognitive Behavioral Therapy for Depression: A Case Report

Abstract

This article examined the effects of a brief, guided Internet-Based Cognitive Behavioral Therapy (iCBT) program on depression through a case study. Extensive research supports the efficacy of iCBT in managing symptoms of depression and anxiety disorders. However, this study specifically focused on the impacts of guided and short-term iCBT intervention on depression symptoms. The case involved an adult female with moderate depressive symptoms participating in a guided iCBT intervention, and the results of this process were detailed. The intervention program utilized in this case includes 15 online modules containing cognitive-behavioral techniques tailored for depression, complemented by six in person sessions. Core skills such as psychoeducation, behavioral activation, identification of automatic thoughts, cognitive restructuring, intermediate and core belief modification, and relapse prevention were taught in the modules and reinforced during the sessions. Depression levels were monitored throughout the process using the Beck Depression Inventory (BDI). Initially, the BDI score was measured at 22, indicating moderate depression, and it decreased to 0 by the end of the intervention, with substantial improvements observed in the client’s overall quality of life and daily functioning. This case study suggests that brief, guided iCBT holds potential as an effective method for reducing depressive symptoms and highlights its value as a tool for addressing existing limitations in access to psychotherapy.
