Examining Positive Emotions from the Perspective of Object Relations in Depressive Personalities

Abstract

The current paper focuses on the conceptualization of depression from Kleinian perspective, which had a great influence on the development of object relations theories. In this frame, Klein’s definitions of paranoid-schizoid position and depressive position are discussed and the developmental processes, associated defense mechanisms, and functioning for healthy resolution of these positions are presented. Moreover, the relationship of these issues to depressive symptomatology is examined with some supporting empirical data. Following, common transferential and countertransferential reactions in working with depressive patients, especially positive transference and idealizing transference are introduced. Throughout the text, these theoretical discussions about the relationship between object relational functioning, transference and countertransference reactions and depressive symptomatology is clarified with the presentation of a case who had depressive symptoms as a result of recurrent losses since her childhood and whose therapeutic process has been conducted in AYNA Clinical Psychology Unit for nearly one year. The paper emphasizes that the assessment of object relational functioning as well as the transferential and countertransferential reactions while working with depressive patients provide important source of information about the therapeutic process.

Depresif Kişiliklerle Psikoterapide Olumlu Duyguların Nesne İlişkisel İncelenmesi

Abstract

Bu makale nesne ilişkileri teorilerinin gelişiminde önemli bir etkisi olan Kleincı bir bakış açısıyla depresyonun kavramsallaştırılmasına odaklanmaktadır. Bu çerçevede Klein’ın paranoid-şizoid konum ve depresif konum tanımlamaları tartışılmakta ve bu konumların gelişim süreçleri, içerdiği savunma mekanizmaları ve sağlıklı çözümlenebilmesi için gereken işleyişler sunulmaktadır. Ayrıca, bunların depresif semptomatolojiyle olan ilişkisi destekleyici bilimsel bulgularla değerlendirilmektedir. Sonrasında depresif hastalarla çalışırken sıklıkla ortaya çıkan aktarım ve karşı aktarım tepkileri, özellikle olumlu aktarım ve yücelten aktarım tanıtılmaktadır. Nesne ilişkisel işleyiş, aktarım ve karşı aktarım tepkileri ve depresif semptomatoloji arasındaki ilişkiye yönelik teorik tartışmalar, metin boyunca yaklaşık bir senedir AYNA Klinik Psikoloji Ünitesi’nde takip edilen ve çocukluktan itibaren yaşadığı kayıplar sonucunda depresif bir kişilik örgütlenmesi gösterdiği düşünülen bir vakanın sunumuyla örneklendirilmektedir. Metin boyunca, depresif kişilik örgütlenmesine sahip hastalarla yapılan çalışmalarda, nesne ilişkisel işleyiş göz önünde bulundurularak terapi sürecinde ortaya çıkan aktarım ve karşı aktarım tepkilerinin değerlendirilmesinin devam eden terapötik sürece yönelik önemli bir bilgi kaynağı olduğu vurgulanmaktadır.
