Beslenme ve Yeme Bozukluğu Eş Tanılı Bir Obsesif Kompulsif Bozukluğun Bilişsel Davranışçı Tedavisi: Bir Olgu Sunumu

Abstract

Bu vaka çalışmasında G. isimli danışanın 10 seans süren psikoterapi ve süpervizyon süreci ele alınmıştır. Vaka çalışmasının ilk bölümünde danışanın demografik bilgileri, yakınmaları, genel durumu, kişisel ve sosyal tarihçesi ele alınmıştır. Elde edilen bilgiler ışığında tanısal değerlendirme amacıyla psikolojik testler uygulanmıştır. Değerlendirme sonrasında olası tanılar ele alınarak ayırıcı tanıya ulaşılmıştır. Danışanın tanısal değerlendirilmesinde Obsesif Kompulsif Bozukluk (OKB) ve Beslenme ve Yeme Bozukluğu (BYB) tanıları ele alınmıştır. Terapist ile danışan arasında terapötik ittifakın kurulması sonrasında danışanın problemlerinin ortaya çıkışı ile ilgili olarak hipotezler oluşturulmuştur. Bilişsel davranışçı terapi teknikleri ile müdahaleler gerçekleştirilmiştir. Bu müdahaleler ayrıntılı olarak ele alınmış ve müdahalelerin tedaviye katkısı psikolojik testler ile değerlendirilmiştir. Son olarak danışanın terapi sürecindeki kazanımları ve
olası prognozu açıklanmıştır. Bu çalışma ile tanısal değerlendirmenin ve psikolojik testlerin tedavi sürecine olan doğrudan etkisi incelenmiştir. Böylece danışanın yakınmaları, psikolojik testler ve tanısal değerlendirmeyi içeren formülasyonun psikoterapi sürecine katkısı incelenerek alanyazına katkı sağlaması amaçlanmıştır.

Cognitive-Behavioral Treatment of an Unspecified Feeding and Eating Disorder with Obsessive-Compulsive Disorder Pattern: A Case Report

Abstract

In this case study, the psychotherapy and supervision process of the patient named G., which lasted 10 sessions, was discussed. In the first part of the case study, the patient's demographic information, complaints, condition, personal and social history were examined. In light of the information obtained, psychological tests were applied for diagnostic evaluation. After the evaluation, possible diagnoses were addressed and the differential diagnosis was achieved. The diagnoses of Obsessive-Compulsive Disorder (OCD) and Eating Disorder (ED) were discussed in the differential diagnostic evaluation of the patient. After establishing the therapeutic alliance between the therapist and the patient, hypotheses were formulated regarding the emergence of the patient's problems. Interventions were performed with cognitive behavioral therapy techniques. These interventions were discussed in detail, and the contribution of interventions to treatment was evaluated with psychological tests. Finally, the patient's gains and possible prognosis in the therapy process are explained. The direct effect of diagnostic evaluation and psychological tests on the treatment process was investigated with this study. Thus, it was aimed to provide a different perspective in the literature by examining the contribution of the patient's complaints, psychological tests, diagnostic evaluation, and the formulation reached in the light of these in the psychotherapy process as a whole.
