Susceptibility of Escherichia Coli Strains Isolated From Urine Cultures to Some Commonly Used Antibacterial Agents

Öz

Objectives: The present study was designed to determine the bacteria causing urinary tract infections (UTI) in our hospital and their susceptibility to various antibiotics. Study Design: Susceptibility of 259 uropathogen Escherichia Coli (E. Coli) isolates from urine samples of patients treated in the wards and intensive care units or in the outpatient clinics of our hospital between March 2004 and February 2005, to ampicillin, amoxycillin-clavulanate, gentamicin, ceftriaxone, ofloxacin, ciprofloxacin, co-trimoxazole, and nalidixic acid, the most commonly used antibiotics in UTI, were evaluated. Among the isolated strains, 63 were from nosocomial and 196 were from community acquired UTI. Identification and susceptibility of strains were determined by an automatized VITEK-1 panel (Biomérieux, France). Results: Comparison of antibiotic susceptibility rates between nosocomial and community-acquired UTI revealed the following results: ampicillin 63-32%, amoxycillin- clavulanate 78-58%, gentamicin 89-83%, ceftriaxone 94-89%, ofloxacin 82-59%, ciprofloxacin 81-54%, cotrimoxazole 71-47%, nalidixic acid 86-64%, nosocomial and community-acquired UTI, respectively. Conclusion: In our opinion, for our region, ciprofloxacin would be the appropriate option for ampiric therapy in patients with probable community-acquired UTI and ampiric parenteral treatment with ceftriaxone would be the appropriate first option in patients with probable nosocomial UTI. Turkish Başlık: İdrar Kültürlerinden İzole Edilen Escherichia Coli Suşlarının Sık Kullanılan Antibakteriyellere Karşı Duyarlılıkları Anahtar Kelimeler: Antibakteriyel ilaçlar; escherichia coli; üriner sistem enfeksiyonu Amaç: Çalışmamızda hastanemizde üriner sistem enfeksiyonu (ÜSE) etkeni olan bakterilerin ve antibiyotik duyarlık durumunun belirlenmesi amaçlandı. Çalışma planı: Mart 2004-Şubat 2005 tarihleri arasında hastanemizde servis ve yoğun bakım birimlerinde yatan ve polikliniklere başvuran hastalardan alınan idrar örneklerinden izole edilen 259 Escherichia coli (E. coli) suşunun üriner sistem enfeksiyonlarında en sık kullanılan antibiyotikler olan ampisilin, amoksisilin-klavulanik asit, gentamisin, seftriakson, ofloksasin, siprofloksasin, trimetoprim- sulfametoksazol ve nalidiksik aside duyarlılıkları araştırıldı. Suşların 63'ü nozokomiyal, 196'sı toplum kökenli üriner sistem enfeksiyonlarından izole edildi. Tanımlama ve duyarlılık testleri VITEK-1 otomasyon sisteminde (Bio mérieux, Fransa) belirlendi. Bulgular: Toplum kökenli ve nozokomiyal ÜSE arasındaki karşılaştırmada antibiyotik duyarlılık oranları sırasıyla ampisilin için %63-32, amoksisilin-klavulanik asit için %78-59, gentamisin için %89-83, seftriakson için %94-89, ofloksasin için %82-59, siprofloksasin için %81-54, trimetoprim-sulfametoksazol için %71-47 ve nalidiksik asit için %86-64 olarak saptandı. Sonuç: Bölgemiz için toplum kökenli ÜSE düşünülen hastalarda ampirik tedavide siprofloksasin, nozokomiyal ÜSE düşünülen hastalarda ise ampirik parenteral tedaviye seftriakson ile başlanmasının uygun olacağı sonucuna varıldı.
