Toll-like Receptor 7 and 9 Gene Polymorphism in Systemic Lupus Eritomatozus Patients

Abstract

Eritomatozus Systemic Lupus (SLE) is a chronic autoimmune disease in which immune system attacks against various cells and organs leading to inflammation, tissue damage and dysfunction. As with all autoimmune diseases in SLE pathophysiology genetic and environmental factors play important roles. In the last few years, studies indicates that Toll-like receptors (TLR) that play an important role in regulation of adaptive immune response to endogenous nucleic acids may be responsible fort he pathophysiological mechanism of SLE. In this study, we aimed to investigate whether there is a significant association between TLR 7 and 9 genotypes and susceptibility to SLE. 53 SLE patients followed by Ege University Faculty of Medicine, Department of Rheumatology and 23 healthy volunteers were included in the study. Patients gender, date of birth, date of SLE diagnosis and weights were determined. Disease activity scored by SLEDAI. Hemoglobin, hematocrit, WBC, platelet counts, complement levels (C3, C4), ANA, dsDNA, anticardiolipin (ACA), IgM, ACA IgG, anti-beta 2 glycoprotein (anti-B2), IgA, IgM and IgG levels were determined from outpatient records. Kidneys, skin and central nervous system involment is evaluated. TLR7 and TLR9 polymorphism analysis was performed by using fluorescent-labeled probes including kits for LightCycler device. Genotypes were identified with melting curve analysis. Statistical analysis was performed by ANOVA, Kruskal-Wallis and Mann-Whitney U test for numeric data and Chi square analysis, Fisher’s exact test for categorical variables. 90.56% of the patients were women (n=48) and 9.44% (n=5) were male. The mean age of patients was 42.17 ± 13, mean disease duration was 9.5 ± 8.1 years.86.95% of the controls were females (n=20), 13.05% were males. (n=3). The control group average age was calculated as 40.13 ± 10.40. In patient group 16 (30.2%) of patients showed TLR9 polymorphism. TLR9 polymorphism was found in 6 (26.1%) person in the control group. In the patient group, 14 (26.9%) patients showed TLR7 polymorphism. In the control group, 9 (40.9%) persons were polymorphic for TLR7. Control and patient group were compared for TLR7 and TLR9 polymorphisms. Results were not statistically significant. TLR7 and TLR9 play important roles in SLE pathogenesis. However there is not a significant association between TLR7 and TLR9 polymorphisms and SLE.

Abstract

Sistemik Lupus Eritomatozus (SLE) hastanın bağışıklık sisteminin vücuttaki çeşitli hücre ve organlara karşı saldırıya geçtiği kronik otoimmün bir hastalık olup çeşitli organlarda inflamasyon, doku hasarı ve fonksiyon bozukluklarına yol açar. SLE’de tüm otoimmün hastalıkların patofizyolojisinde olduğu gibi genetik ve çevresel faktörlerin rolü olduğu belirtilmektedir. Son birkaç yıl içerisinde yapılan çalışmalar adaptif immün yanıt regülasyonunda önemli rol oynayan endojen nükleik asitleri tanıyan Toll-like reseptörlerin (TLR) SLE patofizyolojisinden sorumlu olabileceğine işaret etmektedir. Bu çalışmada SLE hastalarında TLR 7 ve 9 genotipleri ile SLE ‘ye yatkınlık arasında anlamlı bir ilişki olup olmadığının araştırılması amaçlanmıştır. Çalışmaya Ege Üniversitesi Tıp Fakültesi Romatoloji Bilim Dalı tarafından takip edilen 53 SLE hastası ve 23 sağlıklı gönüllü alındı. Hastaların cinsiyetleri, doğum tarihleri, SLE tanısı aldıkları tarih ve kiloları belirlendi. Hastalık aktiviteleri SLEDAI kullanılarak hesaplandı. Hastaların hemoglobin, hemotokrit, lökosit, trombosit değerleri, kompleman düzeyleri (C3, C4), ANA, dsDNA, antikardiyolipin (AKA) IgM, AKA IgG, anti Beta 2 glikoprotein (anti B2) IgA, IgM, IgG düzeyleri poliklinik kayıtlarından belirlendi. Hastalık tutulumları böbrek, cilt ve santral sinir sistemi olarak sınıflandırıldı. TLR7 ve TLR9 polimorfizm analizleri floresan-işaretli problar içeren LightCycler cihazına uygun kit kullanılarak yapıldı.Genotipler “erime eğrisi analizi” (melting curve analysis) ile ayırt edildi. İstatistiksel analizde nümerik veriler için ANOVA, Kruskal Wallis ve Mann-Whitney U testleri kullanıldı. Kategorik değişkenlerin analizinde Ki Kare ve Fischer’s exact test kullanıldı. Hastaların % 90,56’sı kadın (n= 48), % 9,44’ü (n= 5) erkekti. Hastaların yaş ortalaması 42,17 ± 13, hastalık süreleri ortalama 9,5 ± 8,1 yıldı. Kontrol grubunun % 86,95’i kadın (n= 20), % 13,05’i erkekti. (n= 3). Kontrol grubunun yaş ortalaması 40,13±10,40 olarak hesaplandı. TLR9 için hasta grubunda 16 (% 30,2) hastada polimorfizm saptandı. Kontrol grubunda TLR9 polimorfizmi 6 (%26,1) kişide saptandı. Hasta grubunda 14 (% 26,9) hastada TLR7 polimorfizmi saptandı. Kontrol grubunda 9 (% 40,9) kişide TLR7 polimorfizmi saptandı. TLR7 ve TLR9 polimorfizmleri hasta ve kontrol grubunda karşılaştırıldı. İstatiksel olarak anlamlı sonuç saptanmadı. TLR7 ve TLR9’un SLE patogenezinde rol oynadığı bilinmektedir. Ancak TLR7 ve TLR9 polimorfizmleri ve SLE arasında anlamlı bir ilişki saptanmamıştır.
