Renal Hydatid Cyst Mimicking a Bosniak IV Lesion: A Diagnostic Pitfall

Abstract

Introduction: Hydatid disease rarely involves the kidneys and may radiologically mimic malignant renal lesions. We present a rare case of a renal hydatid cyst mimicking a Bosniak IV renal cyst. 

Case Report: A 48-year-old woman was evaluated for an incidentally detected complex cystic mass in the right kidney. Contrast-enhanced computed tomography (CT) demonstrated calcifications and enhancing solid components, leading to a Bosniak IV classification and suspicion of malignancy. The patient underwent laparoscopic partial nephrectomy. Histopathological examination revealed a hydatid cyst, whereas serological tests were negative for hydatid disease. The postoperative course was uneventful.

Conclusion: Renal hydatid cysts may mimic Bosniak IV lesions and lead to misdiagnosis as renal malignancy. In endemic regions, hydatid disease should be considered in the differential diagnosis of complex renal cysts to avoid unnecessary radical surgical interventions.
