The Facial Artery Calcification Detected on Panoramic Radiograph: A Rare Case Report

Abstract

Mönckeberg medial calcific sclerosis is a rare vascular pathology characterized by calcification of the tunica media layer of the arterial wall. This condition is often associated with diabetes mellitus and chronic kidney disease. This case report presents bilateral facial artery Mönckeberg medial calcific sclerosis detected on the panoramic radiograph of a 71-year-old male patient. The patient had a medical history of diabetes mellitus, hypertension, and chronic kidney disease. Panoramic radiography revealed double paralel radiopaque lines in a curved formation resembling a “tram-track calcification” in the bilateral premolar-molar region, below the inferior border of the mandible. The preliminary diagnosis was Mönckeberg medial calcific sclerosis of the facial artery. For a detailed examination, a cone-beam computed tomography scan was performed on the patient, allowing for a three-dimensional examination of the artery’s course, and confirming the diagnosis. The patient was then referred to the cardiology department for evaluation of cardiovascular risk factors and further examination. The identification of arterial calcifications on panoramic radiographs is of critical importance for the early detection of patients with high cardiovascular risk. These radiological findings can contribute to reducing morbidity and mortality by ensuring timely referral of patients to the relevant medical departments.
