Obstetrik Brakiyal Pleksus Yaralanmalı Olgularda Skapular Kinematik Analiz: Vaka Serileri Çalışması

Abstract

Obstetrik Brakiyal Pleksus Yaralanması (OBPY), brakiyal pleksusa bağlı dalların zedelenmesi sonucu ortaya çıkan bir yaralanma şeklidir. İyileşme gerçekleştikçe oluşacak kas imbalansına bağlı olarak ilerleyen yaşlarda deformiteler ve fonksiyonel yetersizlikler meydana gelebilmektedir. OBPY’li olgularda etkilenen taraf skapulanın pozisyonu ve hareketleri etkilenmeyen tarafa göre farklılıklar arz etmektedir. Çalışmanın amacı, OBPY geçiren 3 olguda istirahatte ve kol elevasyonu sırasında skapulanın hareketlerini 3 boyutlu kinematik analiz ile değerlendirmektir. Çalışmaya katılan olguların yaşı sırasıyla 12, 14 ve 11’di. Olguların demografik özellikleri sorgulandıktan sonra Motion Monitor® İskelet Analiz Sistemi ile kol elevasyonu sırasında 3 boyutlu skapular hareketler değerlendirildi. Kol elevasyonu sırasında, tüm olguların skapular hareketi, internal rotasyon, yukarı doğru rotasyon ve posterior tilt, indirme fazında eksternal rotasyon, aşağı doğru rotasyon ve anterior tiltti. Fakat olgularımızın etkilenen ve etkilenmeyen tarafları arasındaki hareketler sırasında senkronizasyon olmadığı belirlendi. Çalışmamıza katılan olgularda skapular diskinezi varlığı ortaya konmuştur. Konuyla ilgili daha net bilgi elde edebilmek ve OBPY sonrası etkilenen taraf kasların diskineziye olan katkılarını inceleyebilmek için çalışmamız, olgu sayısı artırılmış çalışmalarla desteklenmelidir.

Scapular Kinematic Analysis in Children with Obstetric Brachial Plexus Injury: Case Series Study

Abstract

Obstetric Brachial Plexus Palsy (OBPP) is a type of brachial plexus injury. Deformities and functional deficiency may occur in time because of muscle imbalance. The position and motion of the affected scapula may be different from those of the unaffected side. The aim of this study was to investigate scapular motions by three-dimension kinematic analysis during humeral elevation and at rest in three cases. The ages of the cases are 12, 14, and 11 years. Three-dimensional scapular motions were examined during humeral elevation using the Motion Monitor® Skeleton Analysis System after examining other demographic features. Scapular motions of all the cases included internal rotation, upward rotation and posterior tilt during humeral elevation and external rotation, and downward rotation and anterior tilt during humeral depression. However, there was no synchronization between the affected and unaffected scapulas. Scapular dyskinesia was observed in children included our study. To obtain clear data and to investigate the contribution to scapular dyskinesia of the muscles at the affected side, our study should be supported with studies including more cases.
