Triglycerides, Total Cholesterol, and Body Weight Index Demonstrates High Discriminative Performance for Metabolic Syndrome: A Comparative Study with Established Cardiometabolic and Inflammatory Indices
Abstract
Objective: This research sought to assess the effectiveness of combined inflammatory and cardiometabolic markers, with a particular focus on the triglycerides, total cholesterol, and body weight index (TCBI), in individuals diagnosed with metabolic syndrome (MetS). Methods: This retrospective, single-center study included individuals aged 18-65 years who were admitted to the internal medicine clinic between January 2023 and January 2026. MetS was defined according to NCEP ATP III criteria. Demographic, anthropometric, and laboratory data were obtained from electronic medical records. In addition to routine biochemical and hematological parameters, composite indices including TCBI, TyG, CMI, CRP/HDL, NHR, UHR, NLAR, PIV, HALP, NLR, SIRI, and SII were calculated using standard formulas. Appropriate statistical tests were utilized to compare groups. To evaluate the diagnostic effectiveness of the indices, a ROC curve analysis was performed. Results: Several anthropometric and metabolic parameters differed significantly between groups (p < 0.05). Among the evaluated indices, TCBI, CMI, TyG, CRP/HDL, NHR, and UHR levels were significantly elevated in MetS patients (p < 0.0001), whereas NLAR, PIV, HALP, NLR, SIRI, and SII showed no significant differences (p > 0.05). ROC analysis demonstrated that TCBI had the highest diagnostic performance (AUC: 0.9622, 95% CI: 0.9378 - 0.9865, p < 0.0001), followed by CMI (AUC: 0.9600) and TyG (AUC: 0.9463). CRP/HDL, NHR, and UHR exhibited moderate discriminative ability. Conclusion: TCBI showed strong diagnostic performance in this cohort and demonstrated higher overall discriminative ability compared with the evaluated indices. These findings suggest that TCBI may represent a simple and accessible marker for MetS assessment. However, further studies are required to validate its clinical utility.
