THE DECORONATION TECHNIQUE IN THE TREATMENT OF ANKYLOSIS

Abstract

Avulsion is one of the most severe type of dental injuries in permanent teeth. The time of replantation is main determinant of the treatment prognosis. When the periodontal ligament is damaged in the permanent teeth after trauma, the repair process may result in ankylosis. Ankylosis interferes with the vertical growth of the alveolar process, which can lead to an open bite, an unesthetic smile, infraocclusion of anklosed tooth, tilting of the adjacent teeth, elongation of antagonist teeth, and occlusal disharmony. Removal of an ankylosed tooth may be difficult and traumatic leading to esthetic and functional problems for young patients. Recently a treatment option for ankylosed teeth named ‘decoronation’ has gained interest. In this article, in terms of prosthetic rehabilitation which can be done later, recent literature on decoronation demonstrating its importance and clinical benefits has been reviewed.

ANKİLOZ TEDAVİSİNDE DEKORONASYON TEKNİĞİ

Abstract

Avülsiyon daimi dişlerde görülen en ciddi diş yaralanmalarından biridir. Dişin replantasyon zamanı tedavi prognozunun temel belirleyicisidir. Daimi dişlerde periodontal ligamentin travma sonrası hasar gördüğü durumlarda, onarım işlemi dişin ankilozuna yol açabilir. Ankiloz alveoler prosesin büyümesini durdurarak açık kapanışa, estetik olmayan gülüşe, dişin infraoklüzyonuna, komşu dişlerin devrilmesine, antagonist dişlerin uzamasına ve oklüzal düzensizliğe yol açabilir. Ankiloze bir dişin çıkarılması ise zor ve travmatik olup, genç hastalarda estetik ve fonksiyonel pek çok soruna yol açabilir. Son zamanlarda ankiloze dişler için “dekoronasyon tekniği” olarak adlandırılan bir tedavi seçeneği ilgi çekmektedir. Bu makalede, ileriye dönük protez rehabilitasyonu açısından, önemini ve klinik yararlarını gösteren dekronasyon üzerine yayınlanmış güncel literatürler gözden geçirilmiştir.
