Diagnosis Of Asthma In A Misdiagnosed Hemorrhoid Patient With Inappropriate Steroid Use: A Case Report

Abstract

Hemorrhoid is a frequently seen benign anorectal disease. Although its diagnosis is simple, it isfrequently misdiagnosed. Our patient was 55 years old female who had been diagnosed ashemorrhoid and using steroid pomades and suppozituars for 5 years. The patient had alsodyspnea complaints and those medications had help to relieve the symptoms. The patient hadapplied to our clinic for the repetition of her hemorrhoid prescription. Anal examination wasperformed since it was thought from her story that she does not suffer from hemorrhoid. Therewas no sign of hemorrhoid in the anal examination. As the patient had dyspnea complains, shewas referred to Chest Diseases Clinic and she was diagnosed as asthma. The importance ofanorectal examination is well established. But it still sometimes neglected and wrong andunnecessary treatments are given to the patients. In our case, there was an unnecessary treatmentfor hemorrhoid, but the medication helped the treatment of asthma despite its late diagnosis.This case is presented as it has made an interesting contribution to the diagnosis of asthma

Yanlış Hemoroid Tanısı Nedeniyle Uygunsuz Steroid Kullanımı Olan Bir Olguda Astım Tanısı: Olgu Sunumu

Abstract

Hemoroid çok sık görülen benign bir anorektal hastalıktır. Kolay tanınan bir hastalık olmaklabirlikte yanlış tanı konan olgulara sıklıkla rastlanmaktadır. 55 yaşında bayan olan olgumuz 5yıldan beri hemoroid tanısıyla steroidli pomad ve suppozutiar kullanıyordu. Olgu uzun yıllardanberi nefes darlığı çekmekteydi ve hemoroid için verilen ilaçlar sonrası nefesinde belirginrahatlama olmuştu. Kliniğimize, düzenli olarak hemoroid nedeniyle kullandığı ilaçların reçeteedilmesi için başvurdu. Alınan anamnezde hemoridi düşündürecek şikayetleri olmadığıöngörülen hastaya anal muayene yapıldı. Anal muayenede herhangi bir hemoroid bulgusunarastlanmadı. Anamnezde nefes darlığı tarifleyen hasta göğüs hastalıkları ile konsülte edildi vehastaya astım tanısı konuldu. Anorektal hastalıklarda anal muayenenin önemi çok iyibilinmektedir. Buna rağmen anal muayenenin göz ardı edildiği yanlış ve gereksiz tedavilerinuygulandığı olgular sıktır. Bizim olgumuzda da muayene yapılmadığı için gereksiz tedaviuygulanmış neyse ki ilaçlar astım hastalığında faydalı olduğundan astım tanısı gecikmeklebirlikte hasta tedaviden fayda görmüştü. Bu vaka astım tanısını koymaya ilginç bir katkısağladığı düşüncesiyle sunulmuştur
