Evaluation of vancomycin-resistant enterococcus colonization at Duzce Universty research and training hospital

Abstract

Objective: Enterococci exist in normal gastrointestinal flora. Formerly they used to have lowpotential of formation of infection, since 1970s due to the usage of cephalosporins which theyhave a natural resistance; they have an important place among the factors of nosocomialinfection. The aim of this study was to evaluate the status of VRE colonization in DuzceUniversity Research and Training Hospital, Turkey, following a VRE isolation from the bloodsample of a patient (index case). Methods: This study carried out between December 2011 – April 2013 in Düzce UniversityResearch and Training Hospital intensive care and dialysis departments which are consideredas high risk of nosocomial infection. This study carried out in 2 steps. As a first step once aweek and as a second step monthly rectal swaps and environmental samples were collectedfrom all patients of intensive care unit.Results: The rate of VRE colonization was determined in 27 rectal swap sample (6%), 29environmental culture sample (4%). As a precaution, the hospital was close contact insulationprogram. As a results of measures taken, in second step there are no vancomycin-resistantenterococcus detected. With acquisitions of these precautions the rate of vancomycin-resistantenterococcus was decreased rapidly.Conclusion: For the prevention of colonization with VRE in hospitals; the performance ofperiodic surveillance studies, starting with the strict contact isolation programs, in-servicetraining of staff on hand washing, is thought to the importance of ensuring the enforcement ofrestricted use of antibiotics policy and sustainability

DÜZCE ÜNİVERSİTESİ ARAŞTIRMA VE UYGULAMA HASTANESİNDE VANKOMİSİNE DİRENÇLİ ENTEROKOK KOLONİZASYONUNUN DEĞERLENDİRİLMESİ

Abstract

Amaç: Enterokoklar, normal gastrointestinal sistem florasında bulunan bakterilerdir. Geçmişteenfeksiyon oluşturma potansiyelleri düşük iken doğal olarak dirençli oldukları sefalosporinlerinyoğun olarak kullanılmasıyla hastane enfeksiyonu etkenleri arasında önemli bir yer tutmaktadır.Bu çalışmada, Düzce Üniversitesi Araştırma ve Uygulama Hastanesinde yatan bir hastanın(indeks olgu) kan kültüründe VRE üremesi sonrasında, antibiyotik kullanımının yoğun olduğuservislerde yatan hastaların rektal sürüntü örneklerinde vankomisine dirençli enterokok (VRE)kolonizasyonun değerlendirilmesi amaçlanmıştır.Yöntem: Çalışma, Aralık 2011 – Nisan 2013 tarihleri arasında, Düzce Üniversitesi Araştırmave Uygulama Hastanesinde, nozokomiyal enfeksiyon açısından riskli hastane bölümleri olarakkabul edilen yoğun bakım ve diyaliz ünitelerinde yürütülmüştür. İki basamakta yürütülençalışmanın birinci basamağında haftada bir, ikinci basamağında ayda bir kez yoğun bakımünitelerinde bulunan tüm hastalardan, rektal sürüntü örnekleri ve ortam kültürleri alınmıştır.Bulgular: Rektal sürüntü örneklerinin 27(%6)’sinde, ortam kültürlerinin 29 (%4)’unda VREkolonizasyon oranı saptanmıştır. Çalışmanın birinci basamağından sonra önlem olarak,hastanede sıkı temas izolasyon programına başlanmıştır. Alınan önlemler neticesinde yapılanikinci basamak çalışmada VRE suşu saptanmamıştır. Sonuç: Hastanelerde VRE kolonizasyonunun önlenmesi için; peryodik surveyans çalışmalarınınyapılması, sıkı temas izolasyon programına başlanılması, personele el yıkaması konusundahizmet içi eğitim verilmesi, kısıtlı antibiyotik kullanım politikalarının yürürlüğe konulması vesürekliliğinin sağlanmasının büyük önem taşıdığı düşünülmüştür
