Cesarean Section In a Patient With Wolff-Parkinson-White Syndrome Under Spinal Anesthesia

Abstract

Maternal arrhythmias during pregnancy may be life-threatening. Several studies demonstratethat pregnancy predisposes maternal arrhythmias in asymptomatic patients with WolffParkinson-White syndrome. The presence of an accessory pathway may responsible forWolff-Parkinson-White syndrome. Regional anesthesia induced sympathetic blockade causes aprompt decrease in atrial filling pressure and this process results in increased disrhythmia.Autonomic, hemodynamic and hormonal changes may predispose to arrhythmias in pregnancy.Hormonal changes especially abrupt changes in serum levels of estrogens are thought to beresponsible in the development of arrhythmias during pregnancy. This case report describes apregnant woman with Wolff-Parkinson-White syndrome under spinal anesthesia for cesareandelivery surgery

Spinal Anestezi Altında Wolff-Parkinson-White Sendromlu Hastada Sezeryan Ameliyatı

Abstract

Gebelikte olan maternal aritmiler yaşamı tehdit edebilmektedir. Bir çok çalışmada, WolffParkinson-White sendromlu asemptomatik hastalarda gebeliğin maternal aritmilere yatkınlığıarttırdığı bildirilmiştir. Wolff-Parkinson-White sendromundan aksesuar yolların var oluşusorumlu olabilir. Rejional anestezi sırasındaki sempatik blokaj atrial dolum basıncında hızlıazalmaya ve bu da artan disritmilere neden olur. Gebelerdeki otonomik, hemodinamik vehormonal değişiklikler aritmiye yol açabilir. Hormonal değişiklikler, özellikle östrojenin serumseviyesindeki ani değişikliklerin gebelik sırasındaki aritmilerin gelişmesinden sorumluolabileceği düşünülmektedir. Bu olgu sunumunda Wolff-Parkinson-White sendromlu gebe birbayanda spinal anestezi altında sezeryan ameliyatını tartıştık
