Fusion is a developmental anomaly of the dental hard tissue and is defined as the union of adjacent teeth or the union of a normal and supernumerary tooth at some stage in their development. Fusion is rare occurrence and its deffinitive diagnosis is of prime importance for successful root canal treatment. This case report discusses the endodontic management of maxillary central incisor fused with supernumerary tooth which had periapical lesion beyond the apex of the supernumerary tooth. Surgical and nonsurgical endodontic treatment was performed on the supernumerary tooth which had peripical lesion since maxillary left central incisor was intact. After 1 year follow up, no clinical symptoms were present and the maxillary left central incisor remained vital. Recall radiographs and CBCT showed satisfactory healing of periapical pathosis. Proper diagnosis, treatment planning for endodontic management and healing periapical pathosis of fused teeth by using CBCT can ensure predictable and successful results
