Treatment of Skeletal Class III malocclusion in Adult Period by Orthognathic Surgery and Autotransplantation: A Case Report

Abstract

Skeletal Class III malocclusion can be a result of excessive mandibular growth and/or underdevelopment of the maxilla. In the early growth period skeletal Class III malocclusions can be treated by orthodontic and orthopedic appliances, however, in the adult period orthognathic surgery is essential particulaly to correct severe skeletal discrepancies. The main complaint of our patient having skeletal Class III malocclusion presented here was her unfavorable facial esthetics and maxillary retrusion. Treatment plan involved to correct skeletal Class III malocclusion, to obtain a normal overbite and overjet relationship, to autotransplant the impacted maxillary canine, to achieve a stable occlusal relationship, and to improve profile and facial esthetics. After orthodontic levelling, aligning and decompensation of upper and lower dental arches, a 5mm maxillary advancement was achieved with orthognathic surgery utilizing a LeFort I osteotomy. Autotransplantation of the maxillary canine tooth was implemented and prosthetic treatment was used for the esthetic restoration of canine and premolar teeth

Erişkin dönemde iskeletsel sınıf III malokluzyonun ortognatik cerrahi ve ototransplantasyon ile tedavisi: olgu sunumu

Abstract

İskeletsel sınıf III malokluzyonlar, artmış mandibuler büyüme ve/veya maksillanın yetersiz gelişimine bağlı olarak oluşabilirler. Erken büyüme döneminde iskeletsel sınıf III malokluzyonlar ortodontik ve ortopedik apareylerle tedavi edilebilirler, ancak özellikle ağır olguların erişkin dönemde tedavilerinde ortognatik cerrahi kaçınılmazdır. Bu calışmada sunulan iskeletsel sınıf III malokluzyona sahip hastamızın temel şikayeti olumsuz yüz estetiği ve maksiller retruzyondur. Tedavi planı, iskeletsel sınıf III malokluzyonu düzelterek normal overbite ve overjet ilişkisini elde etmek, gömülü maksiller kanin dişi ototransplante etmek, stabil bir okluzal ilişkiyi sağlamak ve profil-yüz estetiğini düzeltmektir. Sabit Edgewise mekanikleri ile alt ve üst dental arkta seviyeleme, sıralama ve dekompenzasyonu takiben LeFort I osteotomi tekniği ile 5 mm maksiller ilerletme elde edilmistir. Gömülü maksiller dişin ototransplantasyonu gerçekleştirilmiş ve daha sonra kanin ve premolar dişlerin estetik restorayonu için protetik tedavi uygulanarak tedavi tamamlanmıştır
