Effects of Class II Malocclusıon Therapy Wıth Four Premolars Extractıons on Thırd Molar Eruptıon

Abstract

The objectives of the study are to evaluate the changes in maxillary and mandibular third molar angulation during active orthodontic treatment with extraction of four premolars in Class ll malocclusions, and to compare changes of upper and lower third molars angulation occured with the change at upper and lower incisors and molars positions. This study was conducted on lateral cephalograms and dental panoramic tomograms DPT of 30 patients mean age 13.48 years with skeletodental Class II malocclusion treated with four first premolar extractions. 24 cases were treated with Edgewise and 6 were treated with Begg technique. The changes of the eruption pattern of the third molars were observed at the begining of the treatment T1 and at the end of the treatment T2 . The changes in antero-posterior position of the incisors and molars, original molar space and mesio-distal third molar angulation were tested by paired-t test. Treatment techniques were compared with Student-t test. Relationship between changes of incisors/first molars position and changes in third molars angulation were tested by correlation analysis. The positional changes of the upper/lower incisors and first molars and original molar space were found similar in Edgewise and Begg technique. Retroclination of the upper incisors and forward movement of the upper first molars were observed. Original molar space was increased in both jaws during orthodontic treatment. Long axis angle of the upper right third molar due to occlusal plane was increased non-significantly. Lower right third molars uprighted significantly. A positive correlation was found between the inclination of the lower right third molars and position of lower incisors and first molars. Posterior structural changes on upper and lower dental arches cannot be determined related to the utilizing method. Because the anchorage is considered to be moderate, available space was gained for all of the third molars eruption. This result validates Class ll malocclusion which occurred in moderate anchorage. Long-term followup have to be observed for the changes in eruption behaviour of the third molars.

DÖRT PREMOLAR ÇEKİMLİ SINIF II MALOKLÜZYON TEDAVİSİNİN ÜÇÜNCÜ MOLAR ERÜPSİYONUNA ETKİSİ

Abstract

Çalışmanın amacı; Sınıf II maloklüzyonun dört premolar çekimli tedavisinde, maksiller ve mandibular üçüncü molar dişlerin angulasyonunun tedavi süresince değişiminin izlenmesi, maksiller ve mandibular keser/molar dişlerin konum değişikliğine bağlı olarak oluşan angulasyon değişiminin incelenmesidir. İskeletsel ve dental Sınıf II maloklüzyona sahip 30 birey ortalama yaş 13.48 yıl çalışmaya dahil edilmiştir. Tedavi için dört birinci premolar diş çekimi planlanan bireylerin lateral sefalometrik ve dental panoramik radyograflarından yarar-lanılmıştır. 24 birey standart Edgewise, 6 birey ise Begg tekniği ile tedavi edilmiştir. Üçüncü molar dişin erüpsiyon paterni tedavi başında T1 ve tedavi sonunda T2 değerlendirilmiştir. Maksiller ve mandibular keser/molar dişlerin anteroposterior konumundaki değişiklik, molar diş için mevcut alandaki ve üçüncü molar dişin mesiodistal angulasyonundaki değişiklik Paired-t test ile ölçülmüştür. Tedavi teknikleri Student-t test ile karşılaştırılmıştır. Keser diş ve birinci molar diş konumlarındaki değişiklik ve üçüncü molar diş angulasyonundaki değişiklik arasındaki ilişki ise korelasyon analiziyle değerlendirilmiştir. Maksiller ve mandibular keser/molar dişlerin konum değişikliği, üçüncü molar diş için mevcut alandaki değişiklik Edgewise ve Begg tekniklerinde benzer bulunmuştur. Maksiller keser dişlerde retroklinasyon, maksiller molar dişlerde ise mesiale migrasyon izlenmiştir. Her iki tedavi tekniğinde de üçüncü molar diş için mevcut alanda artış gözlenmiştir. Maksiller sağ üçüncü molar dişin uzun aksının okluzal planla yaptığı açı artmıştır ancak bu artış istatistiksel olarak önemsiz bulunmuştur. Mandibular sağ üçüncü molar dişte önemli düzeyde dikleşme izlenmiştir. Mandibular sağ üçüncü molar dişin inklinasyonu ve mandibular keser/birinci molar dişlerin konumu arasında pozitif korelasyon bulunmuştur. Dört premolar çekimli tedavi edilen moderate ankrajın kullanıldığı vakalarda üçüncü molar diş erüpsiyonu için yer sağlanmıştır. Üst ve alt dental arklar üzerindeki posterior yapısal değişiklikler kullanılan yöntem ile ilişkilendirilemez. Bu sonuç moderate ankrajın kullanıldığı Sınıf II malokluzyonlarda geçerlidir. Üçüncü molar diş erüpsiyon değişiklikleri uzun dönemde takip edilmelidir.
