Abstract

Diabetes and cancer are significant public health issues in developed and developing countries. Data from the World Health Organization indicate that the prevalence of diabetes is rapidly increasing, and this increase may be associated not only with cardiovascular complications, but also with malignant diseases. Cancer is the second most common cause of death worldwide, and is influenced by environmental, genetic, and lifestyle factors. Epidemiological and molecular studies have shown that certain types of cancers occur more frequently, particularly in patients with type 2 diabetes. In addition to shared risk factors, such as obesity, physical inactivity, advanced age, and dietary habits, diabetes-specific pathophysiological processes, such ashyperinsulinemia, chronic inflammation, and hyperglycemia, may also contribute to the development of cancer. The medications used for diabetes treatment are also important in this context. It has been suggested that metformin may have a protective effect against certain types of cancer, whereas insulin and some insulin analogs may be associated with a potential increase in risk when used long-term. This review aimed to evaluate the multifaceted relationships between diabetes and cancer by examining the current literature and guiding future research.

Diyabet ve Kanser

Abstract

Diyabet ve kanser gerek gelişmiş gerekse de gelişmekte olan ülkelerde önemli halk sağlığı sorunlarıdır. Dünya Sağlık Örgütü verileri, diyabet prevalansının hızla arttığını ve bu artışın yalnızca kardiyovasküler komplikasyonlarla değil, malign hastalıklarla da ilişkili olabileceğini göstermektedir. Kanser ise dünya genelinde ölüme yol açan ikinci en yaygın hastalık grubudur ve çevresel, genetik ve yaşam tarzı faktörlerinden etkilenir. Epidemiyolojik ve moleküler çalışmalar, özellikle tip 2 diyabet hastalarında bazı kanser türlerinin daha sık görüldüğünü ortaya koymuştur. Obezite, fiziksel inaktivite, ileri yaş ve beslenme alışkanlıkları gibi ortak risk faktörlerinin yanı sıra hiperinsülinemi, kronik inflamasyon ve hiperglisemi gibi diyabete özgü patofizyolojik süreçler de kanser gelişimine katkıda bulunabilir. Diyabet tedavisinde kullanılan ilaçlar da bu ilişki açısından önemlidir. Metforminin bazı kanser türlerinde koruyucu olabileceği, insülin ve bazı insülin analoglarının ise uzun süreli kullanımda potansiyel risk artışı ile ilişkilendirilebileceği tartışılmaktadır. Bu derleme, mevcut literatürü inceleyerek diyabet ve kanser arasındaki çok yönlü ilişkileri değerlendirmeyi ve gelecekteki araştırmalara yön vermeyi amaçlamaktadır.
