A Versatile Option for Tip Support, Symmetry, and Soft Tissue Volumization: Medial Crus Caudal Turn-in

Abstract

Objective: The primary objective of this study is to demonstrate the utilization and assess the efficacy of the medial crus caudal turn-in technique in the context of primary septorhinoplasty operations.

Methods: A retrospective analysis was conducted on patients who underwent functional septorhinoplasty between February 2022 and October 2023. Demographic data, primary complaints, findings, preoperative, operative, and postoperative nasolabial angles, and overall satisfaction, measured on the Visual Analog Scale (VAS), were comprehensively evaluated. Both descriptive and inferential statistics were employed for data analysis.

Results: Eighty patients have been found have been operated on by the author. In those 80, the medial crus caudal turn-in technique was used in 8 patients (F : M = 1). Open rhinoplasty was preferred in all cases, and various surgical modalities were utilized, such as classic composite hump reduction, low-strip let-down dorsal preservation, and septal extension grafts. The mean preoperative columellolabial angle was 78°. Postoperatively, mean of this angle increased to 95°. The change was statistically significant (P = .001). Although patients in the tongue-in-groove group saw the biggest decrease in immediate to 3-month postoperative columellolabial angle, the difference was not significant overall (P = .085), and within the subgroups.

Conclusion: The medial crus turn-in emerges as a versatile surgical maneuver with multifunctional applications. It proves effective in providing support and symmetry to the medial crus, reducing columellar show, and volumizing or replacing SMAS (superficial musculoaponeurotic system) dissected away during elevation. This technique has been demonstrated to be both safe and successful, adding to the spectrum of surgical options available in functional septorhinoplasty.
