Oral Focal Mucinosis: A Case Report and Review of the Literature

Abstract

Oral focal mucinosis (OFM) is a rare benign lesion of the oral mucosa, first characterised by Tomich in 1974. It is widely considered the oral counterpart of cutaneous focal mucinosis or myxoid cyst. We present a new representative case of OFM in a young adolescent patient, along with a comprehensive review of the literature from the original description to recent reports. Clinically, OFM typically presents as a slow-growing, painless, pinkish soft-tissue nodule on keratinized mucosa (most often gingiva), usually under 2 cm in size. Radiographs generally show no bony changes. Histologically the lesion is a non‐encapsulated, well-circumscribed myxoid (loosely collagenous) stroma populated by spindle‐shaped fibroblasts embedded in abundant hyaluronic acid–rich mucin. The pathogenesis remains unclear, though local trauma or irritation has been postulated to trigger fibroblast overproduction of hyaluronic acid. Complete surgical excision is curative and recurrences are exceedingly rare. In this case report, a case of oral focal mucinosis diagnosed in a 13-year-old girl is presented with its clinical, radiographic, and histological findings.

Oral Fokal Musinozis: Bir Olgu Raporu ve Literatür Taraması

Abstract

Oral fokal musinozis (OFM), ilk kez 1974 yılında Tomich tarafından tanımlanmış, nadir görülen benign bir oral mukoza lezyonudur. Genellikle, deride görülen fokal musinozis ya da miksoid kistin oral bölgedeki karşılığı olarak kabul edilmektedir. Klinik olarak, OFM en sık keratinize mukozada, özellikle dişetinde ortaya çıkar ve yavaş büyüyen, ağrısız, pembe renkli, yumuşak doku nodülü şeklinde kendini gösterir. Lezyon genellikle 2 cm’den küçük boyuttadır ve yüzeyini örten mukoza normal görünümünü korur. Radyografik incelemelerde kemik dokusunda herhangi bir değişiklik gözlenmez. Histopatolojik olarak lezyon, kapsülsüz ancak iyi sınırlı bir miksoid stromaya sahiptir. Bu stroma, bol miktarda hiyalüronik asit içeren müsin matriksi içerisinde yerleşmiş, iğ biçiminde fibroblast hücreleri ile karakterizedir. Lezyonun patogenezi tam olarak aydınlatılamamış olmakla birlikte, lokal travma ya da kronik irritasyonun fibroblastları uyararak aşırı hiyalüronik asit üretimine neden olabileceği düşünülmektedir. OFM tedavisinde tam cerrahi eksizyon genellikle yeterlidir ve tedavi sonrası nüks oldukça nadirdir. Bu vaka sunumunda , 13 yaşındaki bir kız çocuğunda tespit edilen yeni bir OFM olgusu sunulmakta; ayrıca, Tomich’in orijinal tanımlamasından günümüze kadar geçen sürede literatürde bildirilen OFM vakaları kapsamlı bir şekilde değerlendirilmektedir.
